中文摘要: 目的 分析放射工作人员职业健康体检的结果,了解职业接触电离辐射对工作人员健康的影响。方法 收集2023年某市放射工作人员职业健康检查资料,包括血压、甲状腺B超、心电图以及实验室指标:血常规[白细胞(WBC)、血小板(PLT)、红细胞(RBC)、血红蛋白(HGB)等]、血生化[丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、总胆红素、直接胆红素、总蛋白、白蛋白、空腹血糖(FBG)、肌酐、尿酸等]、甲状腺功能[促甲状腺激素(TSH)、三碘甲状腺原氨酸(T3)、甲状腺素(T4)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)等]项目。结果 收集研究对象共6 977人,男性4 472人,女性2 505人,年龄18~91(34.48±9.99)岁。男性血压、RBC、HGB、FBG、T4、T3、FT3、心电图等指标异常检出率均高于女性(P<0.01);在甲状腺B超、WBC、PLT、TSH等指标上,女性异常率高于男性,差异有统计学意义(P<0.01)。血压、甲状腺B超、FBG、心电图等异常检出率随年龄及工龄的增加而呈现增高的趋势(P<0.01)。血压、甲状腺B超、FBG、心电图等指标出现“离岗后健康检查”组异常检出率急剧上升的趋势。结论 长期接触电离辐射对放射工作人员的健康有一定影响,应重点关注放射工作人员的放射防护,重视其职业健康监护与管理。
Abstract:Objective To analyze the results of occupational health examinations for radiological workers and understand the impact of occupational exposure to ionizing radiation on their health. Methods Data from occupational health examinations in 2023 for radiation workers in a city were collected,including blood pressure,thyroid ultrasound,electrocardiogram and laboratory indicators such as complete blood count[white blood cells(WBC),platelets(PLT),red blood cells(RBC),hemoglobin(HGB),etc.],blood biochemistry[alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin,direct bilirubin,total protein,albumin,fasting blood glucose(FBG),creatinine,uric acid,etc.],thyroid function[thyroid - stimulating hormone(TSH),triiodothyronine(T3),thyroxine(T4),free triiodothyronine(FT3),free thyroxine(FT4),etc.],and other parameters. Results A total of 6 977 subjects were included,with 4 472 males and 2 505 females,aged 18 to 91(34.48 ± 9.99)years. Males had higher abnormaldetection rates than females in indicators including blood pressure,RBC,HGB,FBG,T4,T3,FT3,and electrocardiogram(P<0.01). Females showed higher abnormal rates than males in thyroid ultrasound,WBC,PLT,and TSH,with significant differences(P<0.01). The abnormal detection rates of blood pressure,thyroid ultrasound,FBG,and electrocardiogram increased with the increase of age and seniority(P<0.01). There was a sharp increase in the abnormal detection rate in the "post-work health examination" group for these parameters. Conclusion Long - term exposure to ionizing radiation has certain side effects on the health of radiation workers. It is important to focus on radiation protection for these workers and pay attention to their occupational health monitoring and management.
