Abstract:Objective To observe the clinical efficacy of different treatment courses of Zishen Yutai Pills in interveningspleen -kidney deficiency type diminished ovarian reserve（DOR）infertility patients undergoing in vitro fertilization-embryo transfer（IVF-ET），and to explore the effective treatment duration of Zishen Yutai Pills for DOR infertility IVF?ET. Methods A total of 60 patients with spleen -kidney deficiency type DOR infertility treated in the Reproductive Center and Traditional Chinese Medicine Department，the First People’s Hospital of Foshan from January 2022 to March2024 were selected and randomly divided into three groups（control group，treatment group 1 and treatment group 2），each with 20 cases. The control group received a dietary supplement-yam tablet before the IVF cycle. On the basis of thecontrol group，treatment group 1 started taking Zishen Yutai Pills three menstrual cycles before the IVF cycle，while treatment group 2 started one menstrual cycle before the IVF cycle. Outcomes such as the number of oocytes retrieved，number of high-quality embryos，total gonadotropin（Gn）amount，days of Gn use，and changes in follicle stimulating hormone（FSH），luteinizing hormone（LH），antral follicle count（AFC），and traditional Chinese medicine（TCM）symptom scores were observed before and after treatment. Results Compared with pre-treatment levels, the post-treatment results showed a progressive decrease in FSH and a progressive increase in AFC in the order of treatmentgroup 1, treatment group 2，and control group（P＜0.05），while LH showed a decreasing trend，but the difference was not statistically significant（P>0.05）. In treatment group 1，treatment group 2，and control group，the numbers of oocytes retrieved were 4.00（3.00，5.00），3.00（2.00，4.00），and 2.00（1.00，3.00），respectively，as well as the numbers of high?quality embryos were 2.00（1.00，3.00），2.00（1.00，2.00），and 1.00（0，1.00），respectively，with both treatment groups having a higher number of oocytes retrieved and more high -quality embryos compared to the control group（P<0.05）. There was no significant difference between treatment group 1，treatment group 2 in terms of the number of oocytes retrieved and high -quality embryos（P>0.05）. There was no statistically significant difference in total Gn amount and days of use among the three groups（P>0.05）. After treatment，the TCM symptom score of treatment group 1 was significantly lower than that of treatment group 2［2.00（1.00，3.00）vs 3.00（2.75，4.00），Z=2.996，P<0.05］.Conclusion For patients with spleen?kidney deficiency type DOR infertility，the use of Zishen Yutai Pills during IVF?ET can help improve the quality of ovum and embryo，enhance ovarian function，and alleviate clinical symptoms. Using Zishen Yutai Pills for three menstrual cycles shows more significant therapeutic effects than using them for one cycle.
