中文摘要: 肠道是机体抵御外界病原体及毒素的重要屏障,肠上皮细胞(IECs)及其之间的紧密连接(TJ)构成了肠道屏障的结构基础。在肠道手术中,肠道相关疾病、外科手术操作及缺血再灌注损伤等因素均会对肠道屏障造成不同程度的破坏,致使患者术后并发症增加、恢复时间延长。随着加速康复外科(ERAS)的不断发展,围术期肠道屏障功能的保护对肠道手术的加速康复愈发重要。而胸段硬膜外麻醉(thoracic epidural anesthesia, TEA)具有显著的肠道屏障保护作用,并且易于实施,因而在肠道手术的加速康复中倍受关注。基于ERAS理念下,本文就围术期肠道手术的加速康复策略进行如下综述。
Abstract:Intestinal tract is an important barrier against external pathogens and toxins. Intestinal epithelial cells(IECs) and their tight junctions(TJ) constitute the structural basis of intestinal barrier. In intestinal surgery, factors as intestinal related diseases, surgical procedures and ischemia-reperfusion injury will cause varying degrees of damage to intestinal barrier, resulting in increased postoperative complications and prolonged recovery time. With the continuous development of enhanced recovery after surgery(ERAS), the protection of perioperative intestinal barrier function is becoming more and more important for the enhamced recovery of intestinal surgery.Thoracic epidural anesthesia(TEA) has a significant protective effect on intestinal barrier and is easy to implement, so it has attracted much attention in the enhanced recovery of intestinal surgery. Based on the concept of ERAS, this paper reviews the enhanced recovery strategies of perioperative intestinal surgery as follows.
