中文摘要: 心力衰竭是心血管疾病进展的终末期阶段。射血分数保留型心力衰竭(HFpEF)是心力衰竭较为常见的一种分型,但由于其病理机制较为复杂,临床表型各异,目前关于HFpEF的诊断和治疗仍不是很清楚。研究表明,HFpEF患者往往伴有支链氨基酸(BCAA)水平升高及其代谢紊乱。越来越多的证据显示,高水平的BCAA可通过激活多种信号转导途径影响心脏功能,导致HFpEF的进一步恶化。本文综述BCAA代谢过程及其在HFpEF发病机制中的作用,并总结靶向BCAA代谢过程中的关键酶治疗HFpEF的干预方法,以期为HFpEF的治疗提供新思路。
Abstract:Heart failure(HF)is the end stage of cardiovascular disease. HF with preserved ejection fraction(HFpEF)is a common type of HF,but its diagnosis and treatment are still unclear due to its complex pathogenesis and diverse clinical phenotypes. Research indicates that patients with HFpEF often exhibit elevated levels of branched-chain amino acid(BCAA)and metabolic disturbances.There is increasing evidence that high levels of BCAA can affect cardiac function by activating multiple signalling pathways,leading tofurther deterioration of HFpEF. This article reviews the BCAA metabolism and its role in the pathogenesis of HFpEF,and summarizes the intervention methods for the treatment of HFpEF targeting key enzymes in the BCAA metabolism process,in order to provide new ideas for the treatment of HFpEF.
