中文摘要: 脓毒症是指因感染引起的宿主反应失调导致的危及生命的器官功能障碍,感染性休克是指脓毒症合并严重的循环、细胞和代谢功能紊乱,凝血功能紊乱是其病理生理过程中的关键因素。血栓弹力图(TEG)是反映血液凝固动态变化的指标,与常规凝血指标相比,具有测量时间短、用血少、不受肝素影响的优点。感染性休克患者常预后不良,存在凝血系统激活和炎症反应相互作用,而TEG参数与其炎症指标关系密切。本文阐述了感染性休克患者凝血功能的改变及TEG参数与感染性休克患者的炎症指标的关系,总结TEG在指导感染性休克治疗中的应用,探讨TEG评估感染性休克中的优势和局限性,以寻求更多方式尽早识别感染性休克患者,改善患者预后。
Abstract:Sepsis refers to life-threatening organ dysfunction caused by a host's dysregulated response to an infection, while septic shock is characterized by severe circulatory, cellular, and metabolic dysfunction in combination with sepsis. Disruption of coagulation function is a key factor in the pathophysiological process of septic shock. Thrombelastography (TEG) is an indicator that reflects the dynamic changes in blood coagulation. Compared to conventional coagulation tests, TEG has the advantages of shorter measurement time, minimal blood usage, and no interference from heparin. Patients with septic shock often have a poor prognosis, with activation of the coagulation system and interaction with the inflammatory response. TEG parameters are closely related to inflammatory markers in septic shock patients. This article describes changes in coagulation function in patients with septic shock, the relationship between TEG parameters and inflammatory markers in septic shock patients, summarizes the application of TEG in guiding the treatment of septic shock, discusses the advantages and limitations of TEG in evaluating septic shock, and seeks more ways to identify septic shock patients early and improve patient outcome.
