中文摘要: 目的 探讨对下肢急性深静脉血栓(ADVT)采用AngioJet血栓抽吸系统与导管接触性溶栓(CDT)的治疗效果。
方法 选取内蒙古包钢医院2020年1月至2021年3月收治的30例ADVT患者,按随机数字表法将其分为对照组(n=15)和观察组(n=15),对照组实施CDT治疗,观察组实施AngioJet血栓抽吸系统治疗,比较两组患者的疗效。
结果 观察组血栓清除分级优于对照组(Z=2.340, P=0.019),患肢消肿率(93.33%)高于对照组(53.33%),差异有统计学意义(P<0.05)。观察组溶栓时间[(0.50±0.11)h vs (72.14±0.42)h, P<0.01]、尿激酶用量[(18.51±2.02) ×104u vs (85.34±2.75) ×104u, P<0.01]均低于对照组。观察组并发症发生率(13.33%)稍低于对照组(26.67%),但差异无统计学意义(P>0.05)。
结论 对ADVT患者采取AngioJet血栓抽吸系统治疗较CDT具有更好的疗效,溶栓时间更短,尿激酶用量更少,且临床应用安全性较好。

Abstract:ObjectiveTo investigate the treating effects of acute deep venous thrombosis (ADVT) of lower extremity using the AngioJet thrombectomy system compared to catheter-directed thrombolysis (CDT).
Methods A total of 30 ADVT patients admitted to Baogang Hospital of Inner Mongolia from January 2020 to March 2021 were selected, and randomly divided into control group (n=15) and observation group (n=15). The control group received CDT treatment, while the observation group underwent treatment with the AngioJet thrombectomy system. The treatment outcomes between the two groups were compared.
Results The thrombus removal grade in the observation group was better than that in the control group (Z=2.340, P=0.019), with a limb swelling reduction rate of 93.33% versus 53.33%, respectively, showing a statistically significant difference (P<0.05). The thrombolytic time [(0.50±0.11)h vs (72.14±0.42)h, P<0.01] and the amount of urokinase used [(18.51±2.02) ×104 u vs (85.34±2.75) ×104 u, P<0.01] were both lower in the observation group compared to the control group. The incidence of complications in the observation group (13.33%) was slightly lower than that in the control group (26.67%), but the difference was not statistically significant (P>0.05).
Conclusion The AngioJet thrombectomy system offers better effects for ADVT patients compared to CDT, with shorter thrombolytic time, less urokinase usage, and a better clinical safety profile.
