中文摘要: 目的 探讨采用科普视频的方式对腹股沟疝患儿进行腹腔镜手术术前访视的效果。
方法 选取2020年1月至2022年12月东莞市人民医院外科收治的98例拟行腹腔镜手术的腹股沟疝患儿作为研究对象,以入院时间先后分为观察组和常规组,前49例为常规组,后49例为观察组。常规组实施传统术前访视,观察组采用基于科普视频的术前访视。测量并比较进入手术室时两组患儿心率、血压、呼吸频率等生理应激变化情况;采用改良耶鲁术前焦虑量表(m-YPAS)、麻醉诱导期合作量表(ICC)评估干预方式对患儿术前不同时间点焦虑程度与麻醉诱导期配合度的影响;比较两组家属的护理满意度。
结果 观察组进入手术室时心率及呼吸明显加快、血压迅速升高比例低于常规组,等候期及麻醉前术前焦虑程度低于常规组,差异有统计学意义(P<0.05);患儿麻醉诱导期配合度高于常规组(P<0.05);家属护理满意度高于常规组,差异有统计学意义(95.92%vs 73.47%,χ2=9.525,P<0.05)。
结论 基于科普视频的术前访视有利于减轻行腹腔镜手术腹股沟疝患儿生理应激,缓解焦虑情绪,提升麻醉诱导期配合度,并提高家属对护理方式的满意度。
Abstract:Objective To explore the effectiveness of preoperative visits based on a popular science video in laparoscopic surgery for children with inguinal hernia.
Methods Ninety-eight pediatric inguinal hernia patients scheduled for laparoscopic surgery at Dongguan People's Hospital from January 2020 to December 2022 were selected as the study subjects. They were divided into an observation group and a routine group based on admission time, with the first 49 cases in the routine group and the subsequent 49 cases in the observation group. The routine group underwent traditional preoperative visits, while the observation group received preoperative visits based on popular science videos. Changes in physiological stress, such as heart rate, blood pressure, and respiration rate, were measured and compared upon entering the operating room. The impact of the intervention method on the anxiety level at different time points before surgery and the cooperation during the anesthesia induction period were assessed using the Modified Yale Perioperative Anxiety Scale (m-YPAS) and the anesthesia induction period cooperation (ICC) scale. The satisfaction of family members with the nursing care was compared between the two groups.
Results Compared with the routine group, in the observation group, the incidences of rapid increase of heart rate, respiration rate, and blood pressure upon entering the operating room were significantly lower, the degree of anxiety during waiting period and before anesthesia were lower, and the degree of cooperation during anesthesia induction was higher, with statistically significant differences (P<0.05). Family members' satisfaction with nursing care was higher in the observation group than that in the routine group, with statistically significant differences(95.92%vs 73.47%,χ2=9.525,P<0.05).
Conclusion Preoperative visits based on popular science videos are beneficial for reducing physiological stress and alleviating anxiety, improving cooperation during laparoscopic surgery for pediatric inguinal hernia, and enhancing family satisfaction with the nursing care approach.
