中文摘要: 目的 分析儿童重症腺病毒肺炎(SAP)的危险因素,为临床早期识别SAP提供参考依据。
方法 回顾性选取2010年1月至2019年12月在江西省儿童医院住院的1046例腺病毒肺炎患儿为研究对象,其中SAP组705例,非SAP组341例。收集两组的性别、年龄、发病季节、是否为早产或低出生体重儿、有无基础疾病、临床表现、实验室检查等病例资料,采用单因素和多因素分析找出与SAP有关的危险因素。
结果 单因素分析结果显示,与非SAP组相比,SAP组患儿热程≥1周(73.9% vs 66.3%)、丙氨酸氨基转移酶(ALT)>100u/L(21.4% vs 9.1%)以及混合细菌感染发生率(10.8% vs 3.5%)均显著增高(P<0.05)。多因素logistic回归分析显示,ALT>100u/L、混合细菌感染是发生SAP的独立危险因素(P<0.01)。
结论 ALT>100u/L、混合细菌感染显著增加腺病毒肺炎发生重症的风险,对存在上述特征的腺病毒肺炎患儿应早期积极干预。

Abstract:Objective To explore the risk factors of severe adenovirus pneumonia(SAP) in children and provide reference basis for early identification of SAP in clinic.
Methods A total of 1046 children with adenovirus pneumonia(AP) admitted to Jiangxi Provincal Childrens Hospital from Januery 2010 to Pecember 2019 were selected for a retrospective analysis. There were 705 cases of SAP(SAP group) and 341 cases of non-SAP(NSAP group). The clinical data,including gender, age, onset season, history of premature or low birth weight infants and underlying diseases, clinical manifestations and laboratory examinations were collected in two groups to find out the risk factors for SAP by the univariate analysis ond multivariate analysis.
Results Univariate analysis showed that the incidences of fever time ≥1 week(73.9% vs 66.3%), alanine aminotransferase(ALT) level of >100u/L(21.4% vs 9.1%) and co-infection bacterial(10.8% vs 3.5%) in SAP group were significantly higher than those in NSAP group(P<0.05). Multivariate logistic regression analysis showed that elevated level of ALT(>100u/L) and co-infection with bacterias were the independent risk factors of SAP(P<0.01).
Conclusion Early and active intervention should be carried out for AP in children accompanied with high level of ALT(>100u/L) and with bacterial.
