中文摘要: 目的 探讨联合应用窄带成像技术(NBI)在内镜黏膜下剥离术(ESD)治疗早期结直肠癌中的临床价值。方法回顾性分析2018年1月至2020年12月于中国医科大学附属第四医院行ESD治疗的101例早期结直肠癌患者的临床资料。根据术前检查方式不同分为两组,NBI组54例,白光内镜(WLE)组47例。对比两组ESD治疗结果、术后病理结果及追加外科手术情况。结果 NBI组术后垂直切缘阳性率低于WLE组(1.9% vs 12.7%,P<0.05),完整切除率高于WLE组(96.3% vs 83.0%,P<0.05),追加外科手术率低于WLE组(1.9% vs 12.7%,P<0.05)。结论 联合应用NBI可有效降低术后垂直切缘阳性率,提高ESD治疗早期结直肠癌的完整切除率,降低追加外科手术风险。
Abstract:Objective To investigate the clinical value of narrow band imaging (NBI) combined with endoscopic submucosal dissection (ESD) in the treatment of early colorectal cancer (CRC). Methods A retrospective analysis was performed on the clinical data of 101 patients with early CRC undergoing ESD at the Fourth Affiliated Hospital of China Medical University from January 2018 to December 2020. The patients were divided into NBI group (n=54) and white light endoscopy (WLE) group ( n=47) according to different preoperative examination methods. Baseline characteristics, ESD-related indicators, postoperative pathology and additional surgery were compared between two groups. Results In NBI group, the positive vertical margin rate was lower than that in WLE group (1.9% vs 12.7%, P<0.05), the complete resection rate was higher than that in WLE group (96.3% vs 83.0%, P<0.05), and the rate of additional surgery was lower than that in WLE group (1.9% vs 12.7%, P<0.05).Conclusions In the treatment of early CRC, NBI combined with ESD can effectively reduce the positive vertical margin rate and the risk of additional surgery and increase the complete resection rate of tumor.
