中文摘要: 目的 评价用肝脏瞬时弹性(HepaTest)检测的受控衰减参数(CAP)来诊断非酒精性脂肪肝与彩色超声结果的一致性,及CAP与临床指标的相关性。方法 选择2020年1月至12月在深圳市罗湖区人民医院肝病科就诊,同时进行HepaTest检查和上腹部超声检查的患者为研究对象,使用HepaTest检测患者肝脏脂肪CAP、肝脏硬度(LSM),收集年龄、性别、BMI、临床生化指标等资料,采用Kappa检验进行一致性分析,评估CAP与彩色超声结果的一致性,并进行多元线性回归分析,建立回归方程,评估影响CAP的相关因素。结果 本研究纳入患者330例,男性223例,年龄(37±10)岁,腹部彩色超声共检测脂肪肝103例(31.21%),采用HepaTest检测脂肪肝124例。以彩色超声为标准,HepaTest检测脂肪肝的准确度为93.6%,敏感性为100%,特异性为90.7%,阳性预测值为83.1%,阴性预测值为100%。CAP与彩色超声有高度一致性(Kappa系数=0.860,P<0.01);多元线性回归分析CAP(Y)与BMI、尿酸(UA)呈线性相关,回归方程为:Y=122.618+3.751×BMI+0.052×UA。结论 CAP诊断脂肪肝与彩色超声具有一致性,CAP与BMI、UA呈正相关关系。
Abstract:Objective To evaluate the consistency of controlled attenuation parameter (CAP) by transient elastography(HepaTest)and color Doppler ultrasound in the diagnosis of nonalcoholic fatty liver disease (NAFLD) and the correlation between CAP and clinical indicators.Methods The patients admitted to Luohu District People′s Hospital of Shenzhen from January 2020 to December 2020 were selected.Hepatest and upper abdominal ultrasonography were simultaneously performed for all patients.Hepatest was used to detect CAP for liver fat and liver stiffness measurement (LSM).The data of age,gender,BMI and clinical biochemical indexes were collected,Kappa test was used to evaluate the consistency of CAP and ultrasound results.Multiple linear regression analysis and regression equation were used to evaluate the related factors affecting CAP.Results Among 330 patients enrolled,there were 223 males,with an average age of (37±10) years.A total of 103 cases (31.21%) of fatty liver were detected by abdominal color ultrasound, and 124 cases of fatty liver were detected by HepaTest. Using color ultrasound as the standard, the accuracy of HepaTest in detecting fatty liver was 93.6%, the sensitivity was 100%, the specificity was 90.7%, the positive predictive value was 83.1%, and the negative predictive value was 100%.CAP was highly consistent with the results of color ultrasound (Kappa coefficient=0.860,P<0.01).Multiple linear regression analysis showed that CAP(Y) was significantly linearly correlated with BMI and UA,and the regression equation was Y=122.618+3.751×BMI+0.052×UA.conclusions In the diagnosis of fatty liver,CAP is consistent with color Doppler ultrasound and positively correlated with BMI and UA.
