基础LH/FSH比值对多囊卵巢综合征患者IVF-ET妊娠结局的影响

中文摘要: 目的 探讨基础促黄体生成素/卵泡刺激素(LH/FSH)比值对多囊卵巢综合征(PCOS)患者体外受精-胚胎移植(IVF-ET)妊娠结局的影响。方法 回顾性分析2018年6月至2020年6月于山东中医药大学附属医院生殖中心采用促性腺激素释放激素(GnRH)拮抗剂方案行IVF-ET治疗的PCOS患者,根据基础LH/FSH比值将136例患者分为LH/FSH<1组(A组)、1≤LH/FSH<2组(B组)和LH/FSH≥2组(C组),探讨三组患者不同基础LH/FSH比值对超促排卵用药、胚胎实验室结果及妊娠结局的影响。结果 三组BMI、促性腺激素(Gn)总用量比较差异有统计学意义(P<0.01),A组>B组>C组。三组患者获卵数、受精率、优质胚胎率、临床妊娠率比较差异无统计学意义(P>0.05)。结论 PCOS患者的BMI以及应用GnRH拮抗剂方案的Gn总用量随着LH/FSH比值降低而升高,LH/FSH比值较低的PCOS患者有获得更佳IVF-ET临床结局的趋势。
Abstract:Objective To investigate the effect of basal luteinizing hormone (LH)/follicle-stimulating hormone (FSH) ratio on pregnancy outcomes of patients with polycystic ovary syndrome (PCOS) undergoing in vitro fertilization-embryo transfer (IVF-ET). Methods A total of 136 PCOS patients received IVF-ET with gonadotropin-releasing hormone (GnRH) antagonist protocol at Affiliated Hospital of Shandong University of Chinese Medicine from June 2018 to June 2020 were analyzed retrospectively.According to the basal LH/FSH ratio,the patients were divided into group A (LH/FSH<1),group B (1≤LH/FSH<2) and group C (LH/FSH≥2).The influences of different basal LH/FSH ratio on ovulation induction drugs,laboratory results of embryo and pregnancy outcome were analyzed in three groups. Results There were significant differences in BMI and total dosage of gonadotropin (Gn) among three groups (group A>group B>group C,P<0.01).There was no significant difference in the number of eggs retrieved,fertilization rate,high-quality embryo rate and clinical pregnancy rate among the three groups(P>0.05). Conclusion With the decrease of LH/FSH ratio in PCOS patients,BMI and Gn dosage increased,and the patients with lower LH/FSH ratio tend to achieve better clinical outcomes of IVF-ET treatment.
