甲状腺癌超声弹性成像定量参数与病灶组织中MMP-9、ANG-1、AG490、WIP1表达的相关性

中文摘要: 目的 探讨甲状腺癌超声弹性成像定量参数与病灶组织中基质金属蛋白酶-9(MMP-9)、血管生成素-1(ANG-1)、AG490、野生型p53诱导的磷酸酶1(WIP1)表达的相关性。方法 选取2017年6月至2019年6月收治的120例甲状腺癌患者作为观察组,另选取本院同期收治的甲状腺良性结节患者98例作为对照组,均行甲状腺弹性超声检查、病灶组织基因检测,对比病灶组织中MMP-9、ANG-1、AG490、WIP1表达水平,应用Logistic回归分析探究甲状腺癌发生有关影响因素,并对比两组剪切波速度比值、不同剪切波速度比值患者病灶组织中MMP-9、ANG-1、AG490、WIP1表达水平,应用Pearson相关性分析甲状腺癌超声弹性成像定量参数与病灶组织中MMP-9、ANG-1、AG490、WIP1表达的相关性。结果 观察组病灶组织中MMP-9、ANG-1、WIP1表达水平较对照组高,AG490表达水平较对照组低(P<0.05);Logistic回归分析结果显示,病灶组织中MMP-9、ANG-1、WIP1表达水平均为甲状腺癌发生的重要危险因素,AG490表达水平是甲状腺癌发生的保护因素(P<0.05);观察组剪切波速度比值高于对照组(P<0.05);剪切波速度比值≥3.14患者病灶组织中MMP-9、ANG-1、WIP1表达水平高于<3.14患者,AG490表达水平低于<3.14患者(P<0.05);经Pearson相关性分析,甲状腺癌患者超声弹性成像定量参数与病灶组织中MMP-9(r=0.569)、ANG-1(r=0.374)、WIP1(r=0.414)表达水平呈正相关(P<0.05),与AG490(r=-0.675)表达水平呈负相关(P<0.05)。结论 甲状腺癌患者剪切波速度比值呈异常升高状态,与病灶组织中MMP-9、ANG-1、WIP1水平呈正相关,与AG490呈负相关,可为临床评估病灶组织生物学行为、制定针对性治疗方案提供可靠参考依据。
Abstract:Objecitve To investigate the relationship between the quantitative parameters of ultrasound elastography(UE) and the expressions of matrix metalloproteinase-9 (MMP-9), angiopoietin-1 (ANG-1), α-cyano-(3, 4-hydroxy) n-benzylstyrene amine (AG490), wild-type p53-induced phosphatase-1(WIP1) in patients with thyroid carcinoma. Methods A total of 120 patients with thyroid carcinoma treated from June 2017 to June 2019 were selected as observation group, and 98 patients with benign thyroid nodules were served as control group in the same period.All patients underwent UE examination and gene detection of focus tissue.The expression levels of MMP-9, ANG-1, AG490 and WIP1 in focus tissues were compared between two groups.Logistic regression analysis was used to explore the influencing factors of thyroid carcinoma, and the ratio of shear wave velocity(Vs) and the expression levels of MMP-9, ANG-1, AG490 and WIP1 in the patients with different Vs ratio were compared between two groups.The relationships between quantitative parameters of UE and the expressions of MMP-9, ANG-1, AG490 and WIP1 were analyzed by Pearson correlation. Results The expression levels of MMP-9, ANG-1 and WIP1 in observation group were significantly higher than those in control group, and the expression level of AG490 was lower than that in control group (P<0.05).Logistic regression analysis showed that MMP-9, ANG-1 and WIP1 were the important risk factors of thyroid cancer, and AG490 was the protective factor (P<0.05).In the patients with Vs ratio more than 3.14, the expression levels of MMP-9, ANG-1 and WIP1 increased, and AG490 decreased significantly compared with those in patients with Vs ratio lower than 3.14 (P<0.05).Pearson correlation analysis indicated that the quantitative parameters of UE were positively correlated with the expression levels of MMP-9 (r=0.569), ANG-1 (r=0.374) and WIP1 (r=0.414) and was negatively correlated with the expression level of AG490 (r=-0.675) significantly (P<0.05). Conclusion In the patients with thyroid carcinoma, Vs ratio is abnormally elevated and is positively correlated with the levels of MMP-9, ANG-1 and WIP1 and negatively correlated with AG490 level, which can provide reliable reference for clinical evaluation of the biological behavior of the focus tissue and formulation of targeted treatment plan.
