中文摘要: 目的 探讨125I粒子植入联合微波消融(MWA)治疗中晚期非小细胞肺癌(NSCLC)的近期临床疗效和安全性。方法 选取2016年7月至2018年11月确诊治疗的中晚期NSCLC患者80例,随机分为试验组和对照组,各40例。对照组给予常规的标准放化疗方案,试验组则给予125I粒子植入联合微波消融术治疗。分析两组患者近两个月的临床疗效、CT改善值、体力改善状况、术后并发症等相关数据。结果治疗后,两组患者CT值均显著低于治疗前,且试验组显著低于对照组(P<0.01)。治疗后试验组体力改善率高于对照组(65.0% vs 42.5%,χ2=4.073,P=0.044)。试验组患者缓解率显著高于对照组(85.0% vs 62.5%,χ2=5.230,P=0.022)。试验组患者术后肺部感染、胸腔积液、胸痛、发热及胃肠反应等并发症发生率低于对照组(P<0.05,P<0.01)。结论125I粒子植入联合微波消融对NSCLC患者的临床疗效确切,能够改善患者生活质量,且拥有较少的并发症。
Abstract:Objective To investigate the shortterm clinical efficacy and safety of 125I seed implantation combined with microwave ablation (MWA) in the treatment of advanced nonsmall cell lung cancer (NSCLC).Methods Eighty NSCLC patients hospitalized from July 2016 to November 2018 were randomly divided into experimental group and control group (n=40,each).The conventional standard chemotherapy was performed in control group,while 125I seeds implantation combined with MWA was conducted in experimental group.The clinical efficacy,the improvements of CT value and physical strength and postoperative complications were analyzed in two groups two months after treatment.Results After treatment,CT values were significantly lower than those before treatment in two groups and were significantly lower in experimental group than those in control group (P<0.01);the improvement rate of physical strength (65.0% vs 42.5%,χ2=4.073,P=0.044)and the remission rate(85.0% vs 62.5%,χ2=5.230,P=0.022)in experimental group were significantly higher than those in control group.The incidence of postoperative complications such as pulmonary infection,pleural effusion,chest pain,fever and gastrointestinal reaction in experimental group was statistically lower than those in control group (P<0.05,P<0.01).Conclusion In the treatment for NSCLC patients,125I seed implantation combined with MWA has definite clinical effect and can improve the quality of life of patients with fewer complications.
