中文摘要: 目的 探讨经纳米炭标记前哨淋巴结(SLN)示踪在子宫内膜癌手术中的应用,为临床提供更为安全和有效的治疗方案。方法 选取2021年1月至2024年1月在南京江北医院经病理确诊为子宫内膜癌并采用腹腔镜入路行标准子宫内膜癌分期手术的42例患者。术前进行影像学评估确认淋巴结状态,术中均行纳米炭标记SLN示踪,记录SLN的检出情况,术后将切除的淋巴结送病理检查。结果 在42例子宫内膜癌患者中,共有38例患者的在手术过程中成功检测出显黑染色的SLN,SLN检出率为90.48%(38/42),共计165个。165个显影SLN主要分布于髂外(38.79%)、闭孔(26.67%)、髂总(18.79%)、髂内(10.30%)、腹主(5.45%)区域,宫旁区未发现。38例患者中,术后病理检查有10例(26.32%)患者发生淋巴结转移。纳米炭标记SLN示踪技术的灵敏度为100%(10/10),阴性预测值为100%(28/28)。结论 纳米炭标记SLN示踪技术为早期子宫内膜癌的诊断与治疗提供了一种高灵敏度、高阴性预测值的精准工具,有利于子宫内膜癌外科治疗向更加安全、有效的方向发展。
Abstract:Objective To investigate the application of carbon nanoparticle-labeled sentinel lymph node (SLN) tracing in endometrial cancer surgery, providing a safer and more effective treatment option for clinical practice.Methods A total of 42 patients diagnosed with endometrial cancer by pathology and undergoing standard surgical staging for endometrial cancer via laparoscopic approach at Nanjing Jiangbei Hospital from January 2021 to January 2024 were selected. The preoperative imaging assessment was performed to confirm the lymph node status. During surgery, all patients underwent carbon nanoparticle-labeled SLN tracing, and the detection of SLNs was recorded. The excised lymph nodes were sent for pathological examination postoperatively. Result Among the 42 patients with endometrial cancer, SLNs with dark staining were successfully detected in 38 cases during surgery, with a detection rate of 90.48% (38/42), totaling 165 nodes. The imaged SLNs were mainly distributed in the external iliac (38.79%), obturator (26.67%), common iliac (18.79%), internal iliac (10.30%), and abdominal aortic (5.45%) regions, with none found in the parauterine area. Among the 38 patients, postoperative pathological examination revealed lymph node metastasis in 10 cases (26.32%). The sensitivity of the carbon nanoparticle-labeled SLN tracing technique was 100% (10/10), and the negative predictive value was 100% (28/28). Conclusion The carbon nanoparticle-labeled SLN tracing technique provides a precise tool with high sensitivity and negative predictive value for the diagnosis and treatment of early-stage endometrial cancer, which is conducive to the development of endometrial cancer surgical treatment in a safer and more effectine direction.
