Abstract:Chronic disease prevention and control pose significant global public health challenges. Integrating information technology with chronic disease management offers a novel approach to health management that aligns with the evolving demographic and disease patterns in China. This paper reviews advancements in digital medicine for chronic disease management, encompassing the development of big data centers and management platforms, the use of innovative digital tools and internet-based hospitals, integration of telemedicine within tiered diagnosis and treatment systems, and the establishment of online health education and information exchange platforms. Harnessing digital medicine in chronic disease management empowers patients towards self-care, enhances medication adherence, and improves overall management outcomes. While this fusion facilitates solutions for prolonged patient monitoring, practical implementations continue to face numerous challenges.
