中文摘要: 目的 探讨以JCI标准为基础的PDCA循环管理模式对医务人员手卫生依从性及正确率的影响。方法 选取2023年1月至2023年6月首都医科大学附属北京地坛医院医生450人,护士636人,医辅及医技105人,第三方人员528人,共1 719人为研究对象。自2023年1月1日开始实施手卫生依从性监管,统计1—6月手卫生执行正确率及依从率。结果 1—6月手卫生执行正确率及依从率增加(χ2=180.471、169.115, P<0.05),6月手卫生正确率和依从率达至最高,分别为85.09%、86.08%;1—6月接触患者前、清洁无菌操作前、接触患者后、接触污物后、接触患者周围环境后五个时机手卫生正确率及依从率均明显增加(P<0.05),6月手卫生正确率及依从率最高,均保持在80%以上;1—6月护士、医生、第三方、医辅及医技手卫生正确率及依从率均有显著提升(P<0.05)。结论 以JCI标准为基础的PDCA循环管理模式可以有效提高医务人员手卫生执行正确率和依从率,达到JCI标准预期值,从而有效降低医院感染率的发生。

Abstract:Objective To explore the effect of PDCA cycle management mode based on JCI standard on compliance rate and accuracy rate of hand hygiene in medical staff. Methods From January 2023 to June 2023, a total of 1 719 people were selected as study subjects from January 2023 to June 2023 in Beijing Ditan Hospital affiliated to Capital Medical University, including 450 doctors, 636 nurses, 105 medical auxiliaries and medical technicians, and 528 third-party personnel. Since January 1, 2023, the PDCA cycle management based on JCI standards was implemented, and the accuracy rate and compliance rate of hand hygiene execution was counted from January to June. Results The accuracy rate and compliance rate of hand hygiene were significantly increased from January to June (χ2=180.471, 169.115, P<0.05), and the accuracy rate and compliance rate of hand hygiene were the highest in June (85.09% and 86.08%, respectively). From January to June, hand hygiene accuracy and compliance rates were significantly increased before contact with patients, before aseptic operation, after contact with patients, after contact with dirt, and after contact with patients' surrounding environment (P<0.05), the correct rate and compliance rate of hand hygiene were the highest in June, all remaining above 80%; From January to June, the accuracy and compliance rates of nurses, doctors, third-party, medical auxiliaries and medical technicians were significantly increased (P<0.05). Conclusion PDCA cycle management mode based on JCI standard can effectively improve the accuracy and compliance rate of medical staff hand hygiene, and reach the expected value of JCI standard.
