环氧合酶-2、Runt相关转录因子3及生存素在口腔黏膜下纤维性变癌变中的表达

Expression of COX-2, RUNX3 and Survivin in oral submucosal fibrosis carcinogenesis

中文摘要: 目的 研究在口腔黏膜下纤维性变(OSF)癌变中环氧合酶-2(COX-2)、Runt相关转录因子3(RUNX3)、生存素(Survivin)的表达及相关性。方法 选取2018年4月至2020年10月在湖南中医药大学第一附属医院接受手术治疗的OSF癌变患者80例作为OSF癌变组,选取同期口腔颌面外科整形手术者40例作为正常对照组。对手术切除的OSF癌变组癌组织和对照组正常口腔黏膜组织,采用免疫组化染色和酶联免疫吸附试验检测其COX-2、RUNX3、Survivin蛋白表达情况及表达量,分析三者表达量与不同口腔组织、癌变不同病理特征的关系以及三者间的相关性。结果 与正常对照组相比,OSF癌变组COX-2、Survivin表达量升高,RUNX3表达量降低(P<0.01)。在OSF癌变组中,按高分化、中分化、低分化之序,COX-2、Survivin表达量递升,RUNX3表达量递降(P<0.01);按Ⅰ期、Ⅱ期、Ⅲ~Ⅳ期之序,COX-2、Survivin表达量递升,RUNX3表达量递降(P<0.01);与无淋巴结转移相比,有淋巴结转移患者COX-2、Survivin表达量升高,RUNX3表达量降低(P<0.01)。Pearson线性相关性分析结果示,COX-2与Survivin呈正相关(r=0.381,P<0.05),COX-2和Survivin分别与RUNX3呈负相关(r=-0.400,P<0.05;r=-0.373,P<0.05)。结论 COX-2、Survivin在OSF癌变组织中高表达,RUNX3低表达,并与OSF癌变分化程度、临床分期及有无淋巴结转移相关。COX-2与RUNX3之间呈负相关,COX-2与Survivin之间呈正相关,RUNX3与Survivin之间呈负相关,三者可能在OSF癌变中起重要的参与作用。
Abstract:Objective To investigate the expression and correlation of cyclooxygenase-2 (COX-2), Runt related transcription factor 3 (RUNX3) and Survivin in oral submucous fibrosis (OSF) carcinogenesis. Methods A total of 80 patients with OSF canceration who received surgical treatment in the First Affliated Hospital of Hunan University of Chinese Medicine from April 2018 to October 2020 were selected as the OSF canceration group, and 40 patients with oral and maxillofacial plastic surgery in the same period were selected as the normal control group. Immunohistochemistry and ELISA were used to detect the protein expression and level of COX-2, RUNX3 and Survivin in the cancerous tissues of OSF and normal oral mucosa of the control group, respectively. The relationship between the expression of COX-2, RUNX3 and Survivin and different oral tissues, different pathological features of carcinogenesis was analyzed. The correlation between the COX-2, RUNX3 and Survivin was also analyzed. Results Compared with the normal control group, the expression of COX-2 and Survivin in OSF canceration group were higher and the expression of RUNX3 was lower (P<0.01). In the OSF canceration group, the expression of COX-2 and Survivin increased and the expression of RUNX3 decreased in the order of high differentiation, medium differentiation and low differentiation (P<0.01). With the increase of clinical stage, the expression of COX-2 and Survivin increased, and the expression of RUNX3 decreased (P<0.01). Compared with patients without lymph node metastasis, the expression of COX-2 and Survivin were higher and the expression of RUNX3 was lower in patients with lymph node metastasis (P<0.01). Pearson linear correlation analysis showed that COX-2 was positively correlated with Survivin (P=0.381, P<0.05); COX-2 and Survivin were negatively correlated with RUNX3 (r=-0.400, P<0.05; r=-0.373, P<0.05), respectively. Conclusion COX-2 and Survivin are highly expressed in patients with OSF canceration, while RUNX3 is poorly expressed, which are related to the degree of differentiation, clinical stage and lymph node metastasis of OSF canceration. There is a negative correlation between COX-2 and RUNX3, a positive correlation between COX-2 and Survivin, and a negative correlation between RUNX3 and Survivin. COX-2, RUNX3 and Survivin may play an important role in OSF canceration.
