中文摘要: 目的探讨富血小板血浆(PRP)对鼻骨缺损模型兔骨愈合的治疗效果,为临床应用提供实验学参考。方法取24只成年新西兰大白兔,体重2.5~3.0 kg,均为雄性,均制备直径为5 mm、深度2.5 mm的鼻骨圆柱形缺损,随机分为PRP组(A组)和空白组(B组),每组12只。B组不用药物处理骨缺损区;A组用制备的兔自体PRP填充于骨缺损区。在术后4、8、12周进行大体观察、组织学染色(HE染色、Masson染色)观察,以及CT值、骨组织中Ⅰ型胶原蛋白(用免疫荧光法)、血清骨碱性磷酸酶(BALP,用ELISA法)水平测定。结果兔鼻骨缺损模型制作成功,术后两组动物均无感染、死亡。对新生骨组织标本组织学观察结果显示,A组纤维组织数量、血管、成骨细胞明显多于B组,Ⅰ型胶原蛋白阳性染色强度明显强于B组。A组4、8、12周时,其骨缺损区CT值、新生骨组织Ⅰ型胶原蛋白测定值、血清BALP水平均高于B组,差异均有统计学意义(P<0.01)。结论PRP对鼻骨缺损兔模型骨愈合有较强的促进作用。
Abstract:Objective To investigate the therapeutic effect of platelet-rich plasma(PRP) on bone healing of nasal bone defect in rabbit model,and to provide experimental reference for clinical application. Methods Twenty-four adult New Zealand white rabbits weighting 2.5-3.0 kg,all male,were used to prepare nasal bone cylindrical defects with a diameter of 5 mm and a depth of 2.5 mm,and were randomly divided into PRP group (group A,n=12) and blank group (group B,n=12).The bone defect area was not treated with drugs in group B,while the bone defect area was filled with rabbit autologous PRP in group A.Gross observation,histological staining (HE staining,Masson staining)and CT value,type Ⅰ collagen in bone tissue(by immunofluorescence staining) and serum bone alkaline phosphatase (BALP,by ELISA method) levels were performed at 4,8 and 12 weeks after operation. Results The rabbit model of nasal bone defect was made successfully,and there was no infection or death in two groups after operation.The histological observation of newborn bone tissue showed that the number of fibrous tissue,blood vessels and osteoblasts in group A were significantly more than those in group B,and the positive staining intensity of type Ⅰ collagen was significantly stronger than that in group B.At 4,8 and 12 weeks in group A,the CT value of bone defect,the measured value of typeⅠ collagen in newborn bone tissue and the level of serum BALP were significantly higher than those in group B (P<0.01). Conclusion PRP has a strong promoting effect on bone healing of nasal bone defect in rabbit model.
