中文摘要: 目的 探讨宫颈环形电切术(LEEP)和冷刀锥切术(CKC)两种治疗方式对有生育需求的高级别宫颈上皮内瘤变(CIN)患者妊娠结局的影响,为临床治疗提供参考。方法 回顾性分析十堰市人民医院2015年1月至2018年12月,因高级别CIN接受LEEP术或CKC术后有妊娠意愿患者81例的临床资料。根据手术方式分为LEEP组(41例)及CKC组(40例),统计两种方式治疗后患者妊娠率、流产率、早产率、胎膜早破发生率、低出生体重儿发生率、剖宫产率等指标。结果 LEEP组和CKC组妊娠率比较差异无统计学意义(90.2% vs 87.5%,P>0.05)。LEEP组流产率(16.2% vs 37.1%)、早产率(5.4% vs 25.7%)、低出生体重儿发生率(8.1% vs 28.6%)、剖宫产率(13.5% vs 42.9%)低于CKC组,差异有统计学意义(P<0.05,P<0.01)。结论 行CKC术患者流产、早产、低出生体重儿发生率及剖宫产率高于行LEEP术患者。对于有生育要求的高级别CIN患者,可以考虑选择LEEP治疗。

Abstract:Objective To investigate the effects of cervical loop electrosurgical excision procedure (LEEP) and cervical cold knife conization (CKC) on the pregnancy outcome of patients with high-grade cervical intraepithelial neoplasia (CIN) and to provide reference for clinical treatment.Methods A retrospective analysis was performed on the clinical data of 81 patients with pregnancy intention after LEEP or CKC for high-grade CIN from January 2015 to December 2018.The patients were divided into LEEP group (n=41) and CKC group (n=40) according to the operation methods.The pregnancy rate,abortion rate,premature birth rate,premature rupture of membranes,low birth weight infants,cesarean section rate were analyzed and compared between two groups.Results There was no significant difference in pregnancy rate between LEEP group and CKC group (90.2% vs 87.5%,P>0.05).The incidence of abortion rate (16.2% vs 37.1%),premature birth rate (5.4% vs 25.7%),low birth weight infants rate (8.1% vs 28.6%),and uterine delivery rate (13.5% vs 42.9%) in LEEP group were significantly lower than those in CKC group (P<0.05 ,P<0.01 ).Conclusions The incidences of abortion,premature birth,low birth weight and cesarean section in patients with CKC are significantly higher than those in patients with LEEP.For the patients with fertility requirements after being with high-grade cervical intraepithelial neoplasia,LEEP treatment can be preferred.
