中文摘要: 目的 探讨静脉肾盂造影(IVP)、多层螺旋CT尿路造影(CTU)、磁共振尿路造影(MRU)和超声等几种不同的影像学联合检查方法对小儿重复肾的诊断以及病理分型的效能。方法 回顾性分析2016年1月至2017年12月术后证实为重复肾的90例患儿不同影像学检查结果,并对几种联合影像学检查方法得到的确诊率和病理分型正确率进行比较,评价不同影像学联合检查方法的应用价值。结果 IVP联合CTU、IVP联合MRU、超声联合MRU检查确诊率和分型正确率均高于IVP联合B超,差异有统计学意义(P<0.05)。IVP联合CTU、IVP联合MRU、超声联合MRU检查确诊率和分型正确率比较无统计学差异(P>0.05)。结论 对于需要观察肾脏血流且不迫切需要评价肾脏功能的情况下可选取B超与MRU联合检查,对于需要评价肾脏功能且对血流观察不迫切的情况下可选取IVP与MRU联合检查。
Abstract:Objective To investigate the efficacy of intravenous pyelography (IVP),multi-slice spiral CT urography (CTU),magnetic resonance urography (MRU) and ultrasound in the diagnosis and pathological classification of children with duplex kidney.Methods A retrospective analysis was performed on different imaging results of 90 children with duplex kidney confirmed after operation from January 2016 to December 2017.The diagnostic accuracy rate and pathological classification accuracy rate were compared among the several combined imaging methods (IVP and CTU,IVP and MRU,IVP and ultrasound,MRU and ultrasound),and the application values were evaluated in them.Results Compared with the combined imaging method of IVP and ultrasound,the diagnostic accuracy rate and pathological classification accuracy rate significantly increased in combined imaging methods of IVP and CTU,IVP and MRU,MRU and ultrasound ( P<0.05),and there were no significant differences in them among the later three combined imaging methods( P>0.05).Conclusions Joint examination of B ultrasound and MRU can be more useful to observe renal blood flow without needing to evaluate renal function urgently,and priority of IVP and MRU can be given on the opposite condition.
