中文摘要: 目的 探讨浓缩生长因子(CGF)对慢性牙周炎患者即刻种植修复的影响,以期为CGF在口腔种植中的临床应用提供参考。方法 回顾性分析2015年12月至2016年8月95例(101颗患牙)慢性牙周炎接受即刻种植的患者的临床资料,以在种植体-骨间隙内添加Bio-Oss骨粉引导骨再生的47例患者(49颗患牙)为对照组,以在对照组的基础上联合CGF治疗(在种植体-骨间隙内添加Bio-Oss骨粉与CGF)的48例患者(52颗患牙)为观察组。对患者术后10 d内不良反应进行记录,对两组患者手术当天、术后3个月和术后6个月边缘骨吸收量进行比较,比较术后3、6个月改良菌斑指数(mPLI)和改良出血指数(mSBI)。结果 观察组总不良反应率为5.77%,稍低于对照组(16.33%),但差异无统计学意义(P>0.05)。与对照组比较,观察组种植体边缘骨吸收量在术后当天[(0.30±0.09)mmvs (0.41±0.15)mm]、术后3个月[(0.41±0.18)mmvs (0.79±0.25)mm]、术后6个月[(0.46±0.21)mmvs (1.19±0.22)mm]均明显降低(P<0.05)。观察组mPLI在术后3个月(0.62±0.10vs 0.45±0.08)、术后6个月(0.87±0.12vs 0.65±0.09)均明显高于对照组(P<0.05),mSBI在术后3个月(0.70±0.11vs 0.52±0.14)、术后6个月(0.95±0.14vs 0.67±0.12)亦明显高于对照组(P<0.05)。结论 对慢性牙周炎患者在种植体-骨间隙内添加Bio-Oss骨粉的基础上加用CGF行即刻种植修复,具有良好的临床效果,尚需重视定期对患者进行复查及加强口腔卫生的指导。
Abstract:Objective To investigate the effect of concentrate growth factor (CGF) on immediate implant repair in patients with chronic periodontitis to provide reference for the clinical application of CGF in oral implanting. Methods A retrospective analysis was performed for the clinical data of 95 patients (101 teeth) with chronic periodontitis who received immediate implanting from December 2015 to August 2016. The patients were divided into observation group (48 cases, 52 sick teeth) and control group (47 cases, 49 sick teeth) according to the difference of material added in the implant-bone gap. Bio-Oss was added to the implant-bone gap for guiding bone regeneration in control group. CGF and Bio-Oss mixture was added to the implant-bone gap in observation group. The adverse reactions within 10 days after operation were recorded. Marginal bone absorption amounts were compared between two groups on the day of operation, 3 months and 6 months after the operation. The modified plaque index (mPLI) and modified sulcus bleeding index (mSBI) were compared between two groups at 3 months and 6 months after operation. Results The incidence of total adverse reaction in observation group was slightly lower than that in control group, but there was no significant difference (5.77% vs 16.33%, P>0.05). Compared with control group, marginal bone absorption amounts of the implants decreased in observation group on the day of operation [(0.30±0.09)mm vs (0.41±0.15)mm], 3 months[(0.41±0.18)mm vs(0.79±0.25)mm] and 6 months [(0.46±0.21)mm vs(1.19±0.22)mm] after the operation (all P<0.05). The mPLI of 3 months and 6 months after operation in observation group were significantly higher than those in control group(0.62±0.10 vs 0.45±0.08, 0.87±0.12 vs 0.65±0.09, all P<0.05). The MSBI of 3 months(0.70±0.11 vs 0.52±0.14)and 6 months after operation (0.95±0.14 vs 0.67±0.12) were also significantly higher in observation group than those in the control group (all P<0.05). Conclusion The combination adding CGF and Bio-Oss to the implant-bone gap for the immediate implant repair of chronic periodontitis patients has good clinical effect. However, it is still necessary to pay attention to regular reexamination and the guidance of oral hygiene to the patients.
