中文摘要: 1型糖尿病(T1DM)主要由免疫细胞选择性地破坏胰岛β细胞引起。目前,抑制免疫系统攻击或刺激β细胞再生是其主要治疗策略,但这些方法在临床上的效果有限。胰岛移植作为一种有前景的T1DM治疗手段,可能引起局部炎症反应,导致趋化因子和细胞因子的释放,从而吸引并激活免疫细胞,诱发胰岛移植急性排斥反应。该反应可以通过多种方法减缓,包括抑制趋化因子受体的生成、输注调节性T细胞、阻止细胞因子激活、耗竭T细胞以及降低胰岛的免疫原性。本文综述了预防或缓解急性胰岛移植排斥的最新研究动态。
Abstract:Type 1 diabetes mellitus (T1DM) is primarily characterized by the selective destruction of pancreatic islet β cells by immune cells. Currently, suppressing the attack of the immune system or stimulating the regeneration of β cells is the main therapeutic strategy, but the efficacy of these methods is limited. Pancreatic islet transplantation, as a promising treatment for T1DM, may induce local inflammation, leading to the release of chemokines and cytokines that attract and activate immune cells, thereby triggering acute rejection of the transplanted islets. Acute rejection of pancreatic islet transplantation can be mitigated through various approaches, including preventing chemokine receptors production, infusing regulatory T cells, blocking cytokine activation, depleting T cells, and reducing the immunogenicity of pancreatic islet. This review focuses on discussing the latest research developments in preventing or alleviating acute rejection of pancreatic islet transplantation.
