中文摘要: 目的 比较多西环素和阿奇霉素口服治疗难治性肺炎支原体肺炎(RMMP)倾向患儿的效果和安全性。
方法 选取阿合奇县人民医院2023年4月至12月门诊诊治的94例C反应蛋白≥22.86 mg/L的8岁以上MMP患儿为前瞻性研究的研究对象。采用随机数字表法将其分为阿奇霉素组和多西环素组,每组各47例。对比两组患儿治疗5 d后的疗效、72 h内的体温恢复情况及药物不良反应的差异。
结果 多西环素组总有效率高于阿奇霉素组,差异有统计学意义(93.62%vs 73.91%,χ 2=6.672,P <0.05)。阿奇霉素组的不良反应发生率低于多西环素组,但差异无统计学意义( P >0.05),两组患儿均无严重不良反应。治疗后48、72、96、120 h,多西环素组患儿体温恢复正常比例高于阿奇霉素组( P <0.05)。
结论 对于8周岁以上有RMMP倾向的患儿,多西环素在疗效、体温改善方面较阿奇霉素有优势,且安全可靠。

Abstract:Objective To compare the efficacy and safety of oral doxycycline and azithromycin in the treatment of children with a tendency to refractory Mycoplasma pneumoniae pneumonia(RMMP).
Methods A total of 94 MMP children over 8 years old with C reactive protein≥22.86 mg/L who were treated in the outpatient department of Akqi County People’s Hospital from April to December 2023 were selected as the prospective research objects. They were randomly divided into azithromycin group and doxycycline group with 47 cases in each group. The efficacy after 5 days of treatment, body temperature recovery within 72 h, and adverse reactions were compared between the two groups.
Results The total effective rate of the doxycycline group was higher than that of the azithromycin group, with a statistically significant difference (93.62%vs 73.91%,χ 2=6.672,P <0.05). The incidence of adverse reactions in the azithromycin group was lower than that in the doxycycline group, but the difference was not statistically significant ( P >0.05). There were no serious adverse reactions in the two groups. At 48, 72, 96, and 120 hours after treatment, the proportion of the children whose body temperature returned to normal in the doxycycline group was higher than that in the azithromycin group, with a statistically significant difference ( P <0.05).
Conclusion For children over 8 years old with RMMP tendency, doxycycline has advantages over azithromycin in efficacy and temperature improvement, and is safe and reliable.
