结直肠癌患者免疫相关标志物与患者临床病理特征及预后的关系

Relationship between immune-related markers and clinicopathological features and prognosis in patients with colorectal cancer

中文摘要: 目的 分析结直肠癌组织中免疫相关标志物增殖诱导配体/肿瘤坏死因子超家族成员13(APRIL/TNFSF13)、B淋巴细胞刺激因子(BAFF)、淋巴细胞激活基因3(LAG-3)表达与患者临床病理特征和预后的关系。方法 以2020年3月至2021年3月在徐州医科大学附属淮安医院进行腹腔镜结直肠癌根治术治疗的120例结直肠癌患者为对象进行回顾性研究。术后采用免疫组织化学(SP)法检测手术获取的癌组织及癌旁组织标本中APRIL/TNFSF13、BAFF、LAG-3的表达。术后随访3个月,对比不同临床病理特征和预后患者APRIL/TNFSF13、BAFF、LAG-3的表达,分析三者与临床病理特征和预后的相关性。结果 结直肠癌组织中APRIL/TNFSF13(90.83% vs 17.50%, χ2=129.969, P<0.01)、BAFF(85.83% vs 12.50%, χ2=147.590, P<0.01)、LAG-3阳性表达率(84.17% vs 14.17%, χ2=117.633, P<0.01)高于癌旁组织。结直肠癌患者不同病理分期、分化程度、淋巴结转移、预后的APRIL/TNFSF13、BAFF、LAG-3阳性表达率比较,差异有统计学意义(P<0.05)。Logistic回归分析显示,APRIL/TNFSF13、BAFF、LAG-3阳性表达与结直肠癌患者淋巴结转移、病理分期、分化程度及预后情况独立相关(P<0.05)。结论 结直肠癌患者肿瘤组织中APRIL/TNFSF13、BAFF、LAG-3高表达,且三者阳性表达是淋巴结转移、高病理分期、低分化及预后不良的危险因素。
Abstract:Objective To analyze the relationship between immune-related markers [A proliferation-inducing ligand/tumor necrosis factor superfamily 13(APRIL/TNFSF13), B lymphocyte activating factor (BAFF) and lymphocyte activating gene 3 (LAG-3)] and clinicopathological features as well as prognosis in patients with colorectal cancer. Methods A retrospective study was conducted on 120 patients with colorectal cancer who underwent laparoscopic radical resection of colorectal cancer in the Affiliated Huai'an Hospital of Xuzhou Medical University from March 2020 to March 2021. Streptavidin-perosidase (SP) was used to detect the expressions of APRIL/TNFSF13, BAFF and LAG-3 in cancer tissues and adjacent tissue specimens obtained by surgeries. Follow-up for 3 months after operation, the expressions of APRIL/TNFSF13, BAFF and LAG-3 in patients with different clinicopathological characteristics and prognosis were compared, and the correlation between the above three indexes and clinicopathological characteristics as well as prognosis was analyzed. Results The positive expression rates of APRIL/TNFSF13 (90.83%? vs 17.50%, χ2=129.969, P<0.01), BAFF (85.83%? vs 12.50%, χ2=147.590, P<0.01) and LAG-3 (84.17%? vs 14.17%, χ2=117.633, P<0.01) in colorectal cancer tissues were higher than those in adjacent tissues. There were statistically significant differences in the positive expression rates of APRIL/TNFSF13, BAFF and LAG-3 in colorectal cancer patients with different pathological stages, differentiation degrees, lymph node metastasis, and prognosis (P<0.05). Logistic regression analysis showed that the positive expressions of APRIL/TNFSF13, BAFF and LAG-3 were independently correlated with lymph node metastasis, pathological stage, differentiation degree and prognosis in colorectal cancer patients (P<0.05). Conclusion The immune-related markers APRIL/TNFSF13, BAFF and LAG-3 are highly expressed in the tumor tissues of colorectal cancer patients, and their positive expression can be the risk factors for lymph node metastasis, high pathological stage, low differentiation and poor prognosis.
