中文摘要: 目的 探讨基于ABC-X模型的干预管理对老年结直肠癌造口患者心理状态和自护能力的影响,以改善其生活质量。方法选取2020年2月至2022年1月空军军医大学第二附属医院120例老年结直肠癌造口患者为研究对象,按照随机数字表法分为观察组和对照组,各60例。对照组给予常规护理干预,观察组在对照组基础上加以基于ABC-X模型的干预管理,比较干预3个月后两组患者心理状态[焦虑自评量表(SAS)、抑郁自评量表(SDS)及心理弹性量表(CD-RISC)]、自我效能[一般自我效能量表(GSES)]、应对方式[应对方式问卷(SCSQ)]、自护能力[自我能力实施量表(ESCA)]和生活质量[生活质量综合评定问卷(GQOL-74)]情况。结果 干预后,两组SAS、SDS评分、SCSQ消极应对评分低于干预前,且观察组低于对照组(P<0.01);两组CD-RISC评分、GSES评分和SCSQ积极应对评分、ESCA、GQOL-74各维度评分高于干预前,且观察组高于对照组(P<0.01)。结论 基于ABC-X模型的干预管理可显著改善老年结直肠癌造口患者的心理状态、应对方式,增强其自我效能感和自我护理能力,改善患者生活质量。
Abstract:Objective To explore the influence of intervention management based on ABC-X model on the psychological states and self-care ability of elderly patients with colostomy for colorectal cancer.Methods A total of 120 elderly colorectal cancer patients with colostomy in the Second Affiliated Hospital of Air Force Military Medical University from February 2020 to January 2022 were selected as research subjects, and they were divided into observation group and control group according to the random number table method, with 60 cases in each group. The control group was given routine nursing intervention, while the observation group was additionally given ABC-X model-based intervention management based on control group. The psychological states [self-rating anxiety scale (SAS), self-rating depression scale (SDS), connor-davidson resilience scale (CD-RISC)], self-efficacy [general self-efficacy scale (GSES)], coping styles [simplified coping style questionnaire (SCSQ)], self-care ability[exercise of self-care agency scale (ESCA)] and quality of life [generic quality of life inventory-74 (GQOL-74)] were compared between two groups after 3 months of intervention. Results After the intervention, SAS, SDS scores and SCSQ negative coping scores of the two groups were lower than those before the intervention, and those of the observation group were lower than those of the control group (P<0.01); CD-RISC, GSES, SCSQ positive coping score, ESCA and GQOL-74 scores of the two groups were higher than those before the intervention, and those of the observation group were higher than those of the control group (P<0.01).Conclusions Intervention management based on ABC-X model can significantly improve the psychological states and coping styles of elderly patients with colorectal cancer colostomy, enhance the self-efficacy and self-care ability, and improve the quality of life of patients, which is worthy of recommendation.
