中文摘要: 目的 检测急性脑梗死患者血小板聚集功能、血液黏度及纤维蛋白原水平,探讨三者在急性脑梗死疾病过程中的意义。方法选取2020年1月至2021年4月淮安市肿瘤医院收治的52例急性脑梗死患者为研究对象,同时选择同期基线资料与住院急性脑梗死患者相匹配的体检健康者50例为对照组。检测比较急性脑梗死急性期、恢复期和对照组腺苷二磷酸钠盐(ADP)途径诱聚的血小板最大聚集率(MAR)(ADP-MAR)、花生四烯酸(AA)途径诱聚的MAR(AA-MAR)、血液黏度(高切和低切)及纤维蛋白原和部分生化指标。结果 与对照组相比,急性期和恢复期患者ADP-MAR、AA-MAR和血液黏度检测(高切)均显著升高,且急性期患者升高更明显(P<0.05);与对照组相比,急性期患者血液黏度检测(低切)和纤维蛋白原升高(P<0.05)。急性脑梗死患者组血液黏度检测(高切)与ADP-MAR、AA-MAR成正相关(r=0.458、0.241,P<0.05),血液黏度检测(低切)与ADP-MAR、AA-MAR成正相关(r=0.122、0.181,P<0.05)。结论 联合检测血小板聚集率、血液黏度和纤维蛋白原可以帮助临床医师进行急性脑梗死诊断及其他心脑血管疾病的预判,利于及时对高危人群采取相应的防治措施。
Abstract:Objective To explore the significance of platelet aggregation function, blood viscosity and fibrinogen level in patients with acute cerebral infarction. 〖HTH〗〖STHZ〗〖HTH〗Methods〖HTSS〗〖STBZ〗〖WTBZ〗〖HTSS〗 Fifty-two patients with acute cerebral infarction admitted to Huaian Tumor Hospital from January 2020 to April 2021 were selected as the case group, and 50 healthy physical examination subjects with baseline data matching those of hospitalized patients with acute cerebral infarction in the same period were selected as the control group. The maximum aggregation rate (MAR) of platelets induced by sodium adenosine diphosphate (ADP) (ADP-MAR), the MAR induced by arachidonic acid (AA) (AA-MAR), blood viscosity (high shear rate and low shear rate), fibrinogen and some biochemical indexes in the acute phase, convalescent phase and control group were detected and compared. 〖HTH〗〖STHZ〗〖HTH〗Results〖HTSS〗〖STBZ〗〖WTBZ〗〖HTSS〗 Compared with the control group, ADP-MAR, AA-MAR and the blood viscosity test (high shear) were significantly in patients in acute phase and convalescent phase, and the increase was more obvious in patients in acute phase (P<0.05). Compared with the control group, the blood viscosity test (low shear) and fibrinogen increased in patients in the acute phase (P<0.05). In patients with acute cerebral infarction, blood viscosity test (high shear) was positively correlated with ADP-MAR, AA-MAR (r=0.458, 0.241; P<0.05), and blood viscosity at low shear rate was positively correlated with ADP-MAR, AA-MAR (r=0.122, 0.181; P<0.05). 〖HTH〗〖STHZ〗〖HTH〗Conclusion〖HTSS〗〖STBZ〗〖WTBZ〗〖HTSS〗 Combined detection of platelet aggregation rate, blood viscosity and fibrinogen can help clinicians diagnose acute cerebral infarction and predict other cardiovascular and erebrovascular diseases, suggesting that high-risk groups should take corresponding preventive measures.
