中文摘要: 目的 探讨宫颈微环境中Toll样受体4(TLR4)/核因子κB(NF-κB)/诱导型一氧化氮合酶(iNOS)通路在人乳头瘤病毒(HPV)相关宫颈鳞状细胞癌(CSCC)癌变中的差异表达及在CSCC进展中的临床意义。方法 选择2017年10月至2021年12月在山西省儿童医院(妇幼保健院)经病理学确诊的HPV相关的60例宫颈正常(CN)、51例低级别鳞状上皮内病变(LSIL)、60例高级别鳞状上皮内病变(HSIL)以及58例CSCC的妇女为研究对象。采用免疫组织化学SP法检测TLR4、NF-κB、iNOS蛋白表达水平。收集研究对象的人口学特征及癌变相关因素。结果 从CN组、LSIL组、HSIL组到CSCC组,TLR4(1.7%、17.6%、31.7%、55.2%)、NF-κB(1.7%、11.8%、78.2%、89.7%)、iNOS(3.3%、11.8%、31.7%、70.7%)的阳性表达率均呈显著增高,差异有统计学意义(P<0.01);且随宫颈病变的逐步进展,TLR4(趋势χ2=32.900,P<0.01)、 NF-κB(趋势χ2=48.340,P<0.01)、iNOS(趋势χ2=47.862,P<0.01)的表达率逐渐增加。结论 TLR4/NF-κB/iNOS通路的激活可能促进了宫颈鳞状上皮内病变级别的进展及CSCC的发生。
Abstract:Objective To explore the differential expressions of toll like receptor 4 (TLR4) / nuclear factor κB (NF-κB)/inducible nitric oxide synthase (iNOS) pathway in cervical microenvironment in carcinogenesis of human papillomavirus (HPV)-related cervical squamous cell (CSCC), and its clinical significance in the progression of CSCC. Methods Sixty cases of HPV related women with normal cervix (CN), 51 cases of low-grade squamous intraepithelial lesions (LSIL), 60 cases of high-grade squamous intraepithelial lesions (HSIL) and 58 cases of CSCC confirmed pathologically in Shanxi Children's Hospital (Maternal and Child Health Hospital) were selected as the research objects. The expression levels of TLR4, NF-κB and iNOS protein were detected by immunohistochemical SP method. The demographic characteristics and cancer related factors of the subjects were collected. Results From CN group, LSIL group, HSIL group to CSCC group, the positive expression rates of TLR4(1.7%,17.6%,31.7%,55.2%), NF-κB (1.7%,11.8%,78.2%,89.7%) and iNOS (3.3%,11.8%,31.7%,70.7%)increased significantly, and the difference was statistically significant(P<0.01). With the progression of cervical lesions, the expression rates of TLR4 (trend χ2=32.900, P<0.01), NF-κB (trend χ2=48.340, P<0.01) and iNOS (trend χ2=47.862, P<0.01) gradually increased. Conclusion The activation of TLR4/NF-κB/iNOS pathway may promote the progression of cervical squamous intraepithelial lesions and the occurrence of CSCC.
