中文摘要: 目的 探讨儿童脓毒症疾病易感性及进展与白细胞介素-6(IL-6)基因启动子区-572C/G多态性间的关系。方法 回顾性选取2019年5月至2021年5月十堰市妇幼保健院收治的脓毒血症患儿117例为试验组,选取同期体检健康儿童126例作为对照组,并根据第三版脓毒症与感染性休克定义国际共识,将试验组分为脓毒症组57例和脓毒症休克组60例。检测各组受试者外周血血浆中IL-6、肿瘤坏死因子-α(TNF-α)、降钙素原(PCT)水平,采用聚合酶链式反应-限制性内切酶片段长度多态性(PCR-RELP)技术检测IL-6基因rs1800796(-572C/G)位点单核苷酸多态性(SNPs),分析以上基因多态性与脓毒症易感性及其进展的关系。结果 血浆IL-6、TNF-α和PCT水平试验组明显高于对照组(P<0.01),且试验组中脓毒症休克组高于脓毒症组(P<0.01)。IL-6基因位点rs1800796(-572C/G)的基因型CC及等位基因C频率试验组显著低于对照组(P<0.05),且试验组中脓毒症休克组显著低于脓毒症组(P<0.05);IL-6基因-572C/G位点基因型为CC患儿的血浆IL-6、TNF-α和PCT水平低于基因型为CG/GG患儿(P<0.05)。Logisitic回归分析显示,基因型GG是CC型脓毒症患病风险的2.36倍(95%CI: 1.204~4.628,P<0.05)。结论 IL-6基因-572C/G多态性与儿童脓毒症的发病风险及其进展显著相关,CC基因型为脓毒症保护基因,GG基因型是易感基因型,其多态性影响PCT、IL-6和TNF-α的水平。
Abstract:Objective To investigate the associations of susceptibility and progression of sepsis in children with the interleukin-6(IL-6) gene promoter region-572C/G polymorphism. Methods A total of 117 patients with sepsis treated in Shiyan Maternal and Child Health Hospital from May 2019 to May 2021 were assigned as study group, and 126 healthy children were enrolled as control group at the same period.The patients in study group were subdivided into sepsis group(n=57) and septic shock group(n=60) according to the Third International Consensus Definitions for Sepsis and Septic Shock(Sepsis-3). The levels of IL-6,tumor necrosis factor-alpha(TNF-α) and procalcitonin(PCT) in peripheral blood plasma were detected by ELISA, and single nucleotide polymorphisms(SNPs) at rs1800796(-572C/G) of IL-6 gene were measured by polymerase chain reaction restriction endonuclease fragment length polymorphism(PCR-RELP). The associations of the SNPs with the susceptibility and progression of sepsis were analyzed. Results The levels of IL-6, TNF-α and PCT in study group were significantly higher than those in control group(P<0.01), and they in sepsis shock group were higher than those in sepsis group(P<0.01).The frequencies of IL-6 CC genotypes and C allele at-572C/G locus in study group were significantly lower than those in control group(P<0.05),and were also lower in sepsis shock group than those in sepsis group(P<0.05). The levels of IL-6,TNF-α and PCT in children with IL-6 CC genotypes were statistically lower than those in children with IL-6 CG/GG genotypes (P<0.05). Logistic regression analysis showed that the risk of sepsis in patients with GG genotype was 2.36 times higher than that in patients with CC genotype(95%CI: 1.204-4.628,P<0.05). Conclusion IL-6-572C/G gene polymorphism is obviously associated with the progression and outcome of sepsis. The CC genotype may confer a protective effect for sepsis, and the GG genotype is a susceptible genotype and affects the levels of PCT, IL-6 and TNF-α with polymorphisms.
