中文摘要: 目的 探讨股骨近端防旋髓内钉(PFNA)结合线缆环扎内固定治疗老年患者不稳定股骨转子下骨折的有效性。方法 回顾性分析2016年1月至2019年1月厦门大学附属成功医院采用切开复位PFNA结合线缆环扎内固定治疗的16例老年股骨转子下骨折患者的临床资料。对患者一般情况、手术时间、术中出血量、骨折愈合时间以及功能评分、并发症等进行分析。结果 所有患者手术顺利,手术时间(130.56±40.87)min,术中出血(167.50±76.46)ml;所有患者在随访期内达到骨折愈合,愈合时间(3.94±0.57)个月。所有患者无断钉、感染及明显髋内翻畸形发生。结论 对于老年患者不稳定股骨转子下骨折,使用PFNA结合线缆环扎内固定治疗,有利于骨折解剖复位,提高固定的稳定性,术后并发症少,临床效果满意。
Abstract:Objective To investigate the effectiveness of proximal femoral nail anti-rotation (PFNA) combined with cable cerclage fixation in the treatment of unstable subtrochanteric fractures in elderly patients. Methods A retrospective analysis was performed on the clinical data of 16 elderly patients with subtrochanteric fracture of femur treated by open reduction and PFNA combined with cable cerclage fixation in Chenggong Hospital Affiliated to Xiamen University from January 2016 to January 2019. The general condition, operation time, intraoperative blood loss, fracture healing time, functional score, complications of the patients were analyzed. Results All patients were operated on successfully. The operation time was (130.56±40.87) min, and the amount of intraoperative bleeding was (167.50±76.46) ml. All patients achieved fracture healing during the follow-uPperiod, with an average healing time of (3.94±0.57) months and without broken nails, infection and obvious hiPvarus deformity. Conclusion For the elderly patients with unstable subtrochanteric fracture, PFNA combined with cable cerclage fixation is conducive to the anatomical reduction of fracture and the stability of fixation, with fewer postoperative complications and satisfactory clinical effect.
