Abstract:Objective To evaluate the clinical efficacy of extracorporeal membrane oxygenation (ECMO) in the treatment of adult fulminant myocarditis (FM).Methods The clinical data of 3 patients with FM admitted to the ICU,Affiliated Hospital of Nantong University from May 2018 to November 2019 were retrospectively analyzed.The general condition of the patients,the changes of cardiac function,hemodynamics and laboratory indexes after ECMO treatment,fluid balance during ECMO,complications and prognosis were recorded.Results All the 3 patients were complicated with cardiogenic shock and severe arrhythmia,and 1 patient had cardiac arrest.After ECMO adjuvant therapy,hemodynamic status gradually stabilized,vasoactive drugs decreased,myocardial markers and N-terminal B-type natriuretic peptide levels decreased significantly,and cardiac function improved.There was no fluid overload during ECMO adjuvant treatment in all the 3 patients,and there were 2 patients with bleeding at intubation site,and 3 patients with ECMO treatment time of 141 h,151 h and 190 h respectively.ConclusionECMO is one of the effective treatments for adult FM.Early intervention,reasonable flow rate and conservative fluid management strategy can improve the prognosis of patients.
