滋阴养心方对重症手足口病的疗效及患者血清模式识别受体和损伤指标的影响

中文摘要: 目的 探讨滋阴养心方对重症手足口病的疗效及对患者血清模式识别受体炎性因子、器官和损伤指标的影响。方法 随机选择2016年2月至2018年3月治疗的重症手足口病患儿127例。按随机数字表法分为观察组(64例)和对照组(63例),对照组给予阿昔洛韦治疗,观察组给予阿昔洛韦、滋阴养心方治疗,均治疗5 d后观察两组的疗效、血清相关因子水平、不良反应等情况。结果 观察组的总有效率为95.31%,略高于对照组(85.71%),但差异无统计学意义(P>0.05)。观察组的治愈时间、皮疹消退时间、退热时间、口腔溃疡消退时间均短于对照组(P<0.01)。治疗后两组患儿的血清模式识别受体(CD14、Collectin、Dectin)水平均下降,且观察组低于对照组(P<0.01);治疗后两组患儿的炎性因子[白介素(IL)-6、IL-10、C反应蛋白(CRP)、降钙素原(PCT)、肿瘤坏死因子 (TNF)-α]水平均显著下降,且观察组低于对照组(P<0.01),治疗后两组患儿的器官损伤指标[氨基末端B型利钠肽前体(NT-proBNP)、S100β、神经元特异性烯醇化酶(NSE)、肺表面活性物质(SP-A)]水平均显著下降,且观察组低于对照组(P<0.01);两组患儿不良反应比较差异无统计学意义(P>0.05)。结论 阿昔洛韦、滋阴养心方联合治疗重症手足口病的效果较好,可有效缩短治愈时间,改善机体的模式识别受体水平,降低炎症因子水平,从而减少心、脑、肺等脏器的损害,并且不良反应较少。
Abstract:Objective To investigate the effect of Ziyin Yangxin decoction on severe hand-foot-and-mouth disease (HFMD) and its effect on serum pattern recognition receptor and injury index. Methods A total of 127 children with severe HFMD who received treatment from February 2016 to March 2018 were selected and divided into observation group(n=64) and control group(n=63) randomly.The control group was treated with acyclovir, while the observation group was treated with acyclovir and Ziyin Yangxin decoction.After 5 days of treatment, the therapeutic effect, serum related factors levels and adverse reactions of the two groups were observed. Results The total effective rate of the observation group was slightly higher than that of the control group without signifant difference (95.31% vs 85.71%, P>0.05).The cure time, rash regression time, fever abatement time and oral ulcer regression time of the observation group were significantly shorter than those of the control group (P<0.01). After treatment, the levels of serum pattern recognition receptors (CD14, collectin, dectin), inflammatory factors(IL-6, IL-10, CRP, PCT, TNF-α) and organ injury indexes(NT-proBNP, S100β, NSE, SP-A) in the two groups decreased, and those of the observation group were lower than those of the control group(P<0.01). There was no significant difference in adverse reactions between the two groups (P>0.05). Conclusion Acyclovir and Ziyin Yangxin decoction have better effect in the treatment of severe HFMD, which can effectively shorten the cure time, improve the body′s pattern recognition receptor level, reduce the level of inflammatory factors, so as to reduce the damage of heart, brain, lung and other organs, with less adverse reaction.
