自体动静脉内瘘对患者心功能和血压的影响

中文摘要: 目的 探究自体动静脉内瘘对患者心功能和血压的影响,为临床治疗提供参考。方法 选择2016年1月至2017年6月行自体动静脉内瘘成形术并长期进行维持性血液透析的83例患者作为研究对象,入组患者均随访观察2年,分别测定内瘘成形术前及透析后1、3、6、12、24个月心功能和血压水平,比较透析前后患者心功能和血压的变化。结果 患者内瘘形成术前心脏指数(CI)、心输出量(CO)、左室舒张末期容积(LVEDV)、左心室内径(LVD)、左心房内径(LAD)、右心室内径(RVD)、射血分数(EF)均低于使用内瘘透析后(P<0.05),CI、CO于患者透析后1、3、6、12个月呈上升趋势,于透析后24个月下降;EF于患者透析后1、3、6个月呈上升趋势,于透析后12、24个月呈下降趋势;LVEDV、LVD、LAD、RVD于患者透析后1、3、6、12、24个月呈上升趋势。透析后1、3、6、12个月患者收缩压高于内瘘成形术前(P<0.05),透析后24个月收缩压低于内瘘成形术前(P<0.05);舒张压于透析后1、3、6、12、24个月均低于内瘘成形术前(P<0.05),但透析后1、3、6、12个月间无明显变化,透析后24个月呈下降趋势。结论 自体动静脉内瘘的应用影响患者血流动力学,早期可导致心功能代偿性增强,收缩压升高,失代偿时患者心功能下降,收缩压降低,而舒张压持续处于较低水平。
Abstract:ObjectiveTo explore the effect and mechanism of autologous arteriovenous fistula (AVF) on patients′ cardiac function and blood pressure.MethodsEighty-three patients undergoing AVF angioplasty and long-term maintenance hemodialysis from January 2016 to June 2017 were selected as the research subjects.All the patients were followed up for 2 years.Before angioplasty and at 1-,3-,6-,12-,24- month after dialysis,the cardiac function and blood pressure levels were measured respectively,and their changes were compared before and after dialysis.ResultsThere were significant differences in cardiac index (CI),cardiac output (CO),left ventricular end diastolic volume (LVEDV),left ventricular diameter (LVD),left atrial diameter (LAD),right ventricular diameter (RVD),ejection fraction (EF),systolic and diastolic blood pressure levels before operation and after hemodialysis (P<0.05).CI and CO showed an upward trend at 1,3,6,and 12 months after dialysis,and decreased at 24 months after dialysis; EF showed a upward trend at 1,3,and 6 months after dialysis,and showed a downward trend at 12,24 months after dialysis; LVEDV,LVD,LAD,RVD showed an upward trend at 1,3,6,12,and 24 months after dialysis.The systolic blood pressure level within 12 months after dialysis was significantly higher than that before operation(P<0.05) and was significantly lower at 24 months after dialysis than that before operation (P<0.05).Diastolic blood pressure was lower at 1,3,6,12,and 24 months after dialysis than before operation(P<0.05),but there was no significant change during 1,3,6,and 12 months after dialysis,and showed a downward trend at 24 months after dialysis.ConclusionThe application of autogenous arteriovenous fistula affects the hemodynamics of patients,which can lead to the early compensatory enhancement of cardiac function and the increase of systolic blood pressure.However,the cardiac function and the systolic pressure decrease,and the diastolic blood pressure remains at a low level during cardiac decompensation.
