中文摘要: 目的 探讨配对盒基因8(Pax8)与配对盒基因1(Pax1)在宫颈癌组织中的表达及与临床病理特征和预后的关系。方法 选取2014年6月至2016年6月收治80例拟进行手术治疗的宫颈癌患者作为研究对象,将术中切除的癌组织标本纳入观察组,癌旁组织标本(距肿瘤边缘4 cm处)纳入对照组。采用免疫组化法检测两组Pax8与Pax1蛋白的表达。分析Pax8、Pax1蛋白表达与宫颈癌临床病理特征的关系,并采用Cox比例风险回归分析影响宫颈癌患者预后的相关因素。结果 观察组Pax8、Pax1蛋白阳性表达率显著高于对照组(68.75%vs35.00%,70.00%vs36.25%,P<0.01)。Pax8与Pax1蛋白表达在不同宫颈癌肿瘤分期、不同分化程度、有无淋巴结转移、不同浸润深度差异有统计学意义(P<0.05,P<0.01)。随访5~36个月,宫颈癌患者中位生存期21个月,其中Pax8阳性表达者和阴性表达者的中位生存期分别为12、36个月,Pax1阳性表达者和阴性者表达者中位生存期分别为13、34个月,差异有统计学意义(P<0.05)。单因素和多因素分析显示,肿瘤低分化、淋巴结转移、浸润深度≥1/2、肿瘤Ⅲ期、Pax8与Pax1阳性表达均为影响宫颈癌患者预后的独立因素(P<0.05,P<0.01)。结论 Pax8与Pax1蛋白在宫颈癌组织中的表达水平升高,且与宫颈癌患者的临床病理特征有关,Pax8蛋白或Pax1蛋白表达阳性者预后较差,Pax8与Pax1蛋白表达可作为判断宫颈癌预后的生物学指标。
Abstract:ObjectiveTo investigate the expression of paired box (Pax) 8 and Pax1 in cervical cancer tissues,and their correlation with clinicopathological features and prognosis of cervical cancer.MethodsA total of 80 cervical cancer patients who received surgical treatment from June 2014 to June 2016 were selected as the subjects.The 80 cancer tissue samples were selected as observation group and 80 adjacent tissue samples (4 cm from the tumor edge) were selected as control group.The expression of Pax8 and Pax1 protein were detected by immunohistochemistry.The relationship between Pax8 and Pax1 protein expression,and the clinicopathological features of cervical cancer were analyzed.The factors influencing the prognosis of cervical cancer patients were analyzed by Cox proportional risk analysis.ResultsThe positive expression rate of Pax8 and Pax1 protein in observation group were significantly higher than those in control group (68.75% vs 35.00%,70.00% vs 36.25%,P<0.01).There were significant differences in tumor stage,differentiation degree,lymph node metastasis and infiltration depth (P<0.05,P<0.01).The median survival time of cervical cancer patients was 21 months after 5-36 month follow-up.The median survival time of Pax8 positive expression and Pax8 negative expression was 12 months and 36 months,respectively,the median survival time of Pax1 positive expression and Pax1 negative expression was 13 months and 34 months,respectively,the differences were significant(P<0.05).Single-factor and multiple-factor analysis showed that low differentiation,lymph node metastasis,invasion depth≥1/2,tumor stage Ⅲ,Pax8 and Pax1 positive expression were independent factors affecting the prognosis of cervical cancer patients (P<0.05,P<0.01).ConclusionsThe expression level of Pax8 and Pax1 protein in cervical cancer tissue is high,and those are positively correlated with the clinicopathological features of cervical cancer patients.The patients with Pax8 protein or Pax1 protein positive expression may have a poor prognosis.The expression of Pax8 and Pax1 protein could be used as a biological index to judge the prognosis of cervical cancer.
