血清脂多糖和核转录因子-κB与2型糖尿病患者周围神经病变的相关性

中文摘要: 目的 研究2型糖尿病患者血清脂多糖(LPS)、核转录因子-κB (NF-κB)和周围神经病变的相关性。方法 选取2018年4月至2019年2月47例2型糖尿病周围神经病变患者为DPN组,22例2型糖尿病无周围神经病变组为DM组,15名健康体检者为对照组,检测三组患者LPS、NF-κB水平,比较LPS、NF-κB和2型糖尿病周围神经病变的相关性。结果 三组LPS和NF-κB水平比较差异有统计学意义(F=36.587,P<0.01;F=32.787,P<0.01),其中DM和DPN组的LPS和NF-κB水平明显高于对照组(P<0.01),DPN组LPS和NF-κB水平高于DM组(P<0.05)。Pearson相关分析显示DPN组LPS、NF-κB与患者BMI、FBS、HbA1c、空腹C肽、空腹胰岛素、空腹胰高血糖素、胰岛素抵抗指数呈正相关(P<0.05,P<0.01);与HDL-C呈负相关(P<0.01);与年龄、病史、TC、TG、U-Alb无关(P>0.05)。二项Logistic回归分析LPS(OR=1.994,P=0.003)、NF-κB(OR=1.055,P=0.042)是糖尿病周围神经病变的独立危险因素。结论 血清LPS、NF-κB是2型糖尿病周围神经病变的相关因素,其可能通过炎症反应和胰岛素抵抗等参与了糖尿病周围神经病变的发生发展。
Abstract:Objective To study the correlation between serum lipopolysaccharide(LPS),nuclear transcription factor (NF)-κB and diabetic peripheral neuropathy(DPN) in patients with type 2 diabetes mellitus(T2DM). Methods From April 2018 to February 2019,a total of 47 patients with type 2 diabetic peripheral neuropathy were selected as DPN group,22 T2DM patients without neuropathy were selected as DM group,and 15 physical examiners were served as control group.The levels of LPS and NF-κB were detected and compared among three groups to analyze the association of LPS and NF-κB with DPN. b>Results The levels of LPS and NF-κB in three groups were significantly different ( F=36.587,P<0.01; F=32.787,P<0.01).Compared with control group,the serum levels of LPS and NF-κB increased significantly in DM and DPN groups(P<0.01),and they were higher in DPN group than those in DM group(P<0.05).Pearson correlation analysis showed that LPS and NF-κB were positively correlated with BMI,FBS,HbA1c,fasting C peptide,fasting insulin,fasting glucagon and insulin resistance index (P< 0.05,P< 0.01),negatively correlated with HDL-C (P<0.01) and unrelated to age,disease history,TC,TG,U-Alb(P>0.05)in DPN group.Binomial Logistic regression analysis showed that LPS( OR=1.994,P=0.003)and NF-κB( OR=1.055,P=0.042)were the independent risk factors of DPN. ConclusionSerum LPS and NF-κB are the related factors of diabetic peripheral neuropathy and may participate in the occurrence and development of diabetic peripheral neuropathy through inflammatory response and insulin resistance.
