中文摘要: 目的 检测并分析2015-2017年尿培养的细菌分布与体外药敏试验,为临床合理用药提供参考。方法 回顾性分析2015年1月1日至2017年1月1日尿培养的菌种分布与耐药性监测。病原菌菌种的鉴定应用法国梅里埃公司的API鉴定系统,药敏试验采用纸片法(k-b)。结果 1 627例送检的尿培养标本中,共分离出病原菌558株,阳性率34.3%,病原菌中大肠埃希菌数量最多,共256株,占45.9%;屎肠球菌数量67株,占12.0%;肺炎克雷伯菌数量为45株,占7.7%;铜绿假单胞菌数量为37株,占6.6%;粪肠球菌数量为32株,占5.7%。大肠埃希菌产超广谱β-内酰胺酶(ESBL)为63.3%,肺炎克雷伯菌产ESBL为31.5%,肠球菌中未发现万古霉素与利奈唑胺耐药株。结论 导致泌尿系感染的主要病原菌以大肠埃希菌和肠球菌为主,临床应根据药敏结果选用抗菌药物,减少耐药菌株的产生。

Abstract:Objective To detect and analyze the distribution of bacteria in urine culture from 2015 to 2017 and the drug sensitivity test in vitro so as to provide the reference for clinical rational use of drugs.? Methods The bacterial distribution cultured in urine and the monitoring of drug resistance from January 1, 2015 to January 1, 2017 were analyzed retrospectively. The identification of pathogenic bacteria was madeby API identification system from French bioMerieux company. The drug susceptibility test was performed by disk diffusion(k-b) method.? Results Out of 627 urine culture specimens, pathogenic bacteria were isolated from 558 cases (34.3%) in which there were Escherichia coli pathogen (256 strains, 45.9%), being the most common, Enterococcus faecium(67 strains, 12%), Klebsiella pneumoniae (45 strains, 7.7%), copper Pseudomonas aeruginosa(37 strains, 6.6%), Enterococcus faecalis (32 strains, 5.7%). Escherichia coli produced extended spectrum beta lactamase (ESBL) was 63.3%, and ESBL produced by Klebsiella pneumoniae was 31.5%. Vancomycin-and linezolid-resistant strains were not found among staphylococcus.? Conclusion The main pathogens of urinary tract infection are Escherichia coli and Enterococcus, and antibiotics should be selected based on the results of antibiotic susceptibility, so as to reduce the emergence of drug-resistant strains.
