miR-1203对肝细胞性肝癌的转移调控及预后预测

中文摘要: 目的 探讨微小糖核酸(microRNA,miR)-1203与肝细胞性肝癌(HCC)的转移调控及预后的关系。方法 选择2009年7月至2010年10月在南京医科大学第二附属医院住院的80例HCC患者(另8例肝血管瘤、7例肝硬化患者为对照),采用实时荧光定量PCR (qRT-PCR)法检测miR-1203表达,对miR-1203表达量与HCC患者的临床病理特征关系进行分析。从上述95例中选择20例患者的组织标本(其中HCC组13例,4例为未转移HCC,4例为门静脉转移HCC,5例为肝内转移的HCC,3例肝血管瘤患者为正常对照组,4例肝硬化患者为阴性对照组)进行基因测序与筛查,采用共聚分析及韦恩图分析检测差异表达的miRs,分析miR-1203高表达与HCC转移的相关性。体外侵袭实验检测miR-1203对肝癌细胞侵袭作用的影响。 Kaplan-Meier生存分析分析生存率差异。Cox比例回归分析预测HCC患者预后的独立危险因素。结果 临床病理特征分析显示miR-1203表达与HCC血管侵犯、肿瘤分级密切相关(P <0.05,P <0.01)。韦恩图分析及大样本验证表明miR-1203在转移HCC患者组织中明显高表达(P <0.05)。转移性肝癌组患者血清中miR-1203表达量明显增高;Kaplan-Meier生存分析结果显示,miR-1203低表达组的总体生存率及无复发生存率均明显高于miR-1203高表达组,差异均有统计学意义(P均<0.01)。Cox比例回归分析提示miR-1203可作为独立的预测HCC患者预后的标志物(95%CI:1.906~8.784,P <0.01)。结论 miR-1203高表达促进HCC转移,并可提示预后不良。miR-1203可作为一个新的预测HCC预后的临床检测指标。
Abstract:Objective To explore the association of micro RNA(miR)-1203 with the metastasis and prognostic prediction of hepatocellular carcinoma (HCC). Methods Eighty patients with HCC, 8 patients with hepatic hemangioma and 7 patient with liver cirrhosis admitted in the Second Affiliated Hospital of Nanjing Medical University from July 2009 to October 2010 were selected, and real-time fluorescent quantitative polymerase chain reaction (qRT-PCR) was used to detect miR-1203 expression in HCC tissues.The relationship between miR-1203 expression level and the clinicopathologic features of HCC was analyzed.Tissue samples of 20 patients(HCC without metastasis in 4 cases, HCC with portal vein metastasis in 4 case, HCC with intrahepatic metastasis in 5 cases, hepatic hemangioma as normal control in 3 cases, liver cirrhosis as negative control in 4 cases)were collected for gene sequencing and screening.Cluster analysis and Venn diagram analysis were used to detect differently expressed miRs to analyze the correlation between high expression of miR-1203 and metastasis of HCC.Effect of miR-1203 on invasion of hepatocellular carcinoma cells was determined by Transwell invasive experiment in vitro.Kaplan-Meier survival analysis was used to analyze the difference of survival rate.Cox proportional regression analysis was used to predict the independent risk factors of prognosis of HCC patients. Results Clinical pathological features analysis showed that miR-1203 expression was closely correlated with vascular invasion and tumor grading of HCC(P<0.05, P<0.01).Venn diagram analysis and large sample verification showed that miR-1203 presented significant high-expression in metastatic HCC tissues of HCC patients(P<0.05).The expression level of serum miR-1203 significantly increased in metastatic HCC patients.Kaplan-Meier survival analysis showed that overall survival rate and relapse free survival rate in patients with miR-1203 low-expression were all significantly higher than those in patients with miR-1203 higher-expression (all P<0.01).Cox proportional regression analysis suggested that miR-1203 can be served as an independent mark of predicting prognosis of HCC patients (95%CI:1.906-8.784, P<0.01). Conclusion The high expression of miR-1203 might promote the metastasis of HCC and predict poor outcome of HCC patients.miRNA-1203 can be served a new clinical detection index of predicting prognosis of HCC patients.
