中文摘要: 目的 探讨新型经导管心脏瓣膜系统XcorTM应用于健康实验白猪的安全性。 方法 选取20头健康实验用白猪,通过经心尖经导管主动脉瓣置换术植入 XcorTM生物瓣膜。全部受试对象随机分为A、B、C三组,分别于术后即刻(A组,2头)、3个月时(B组,9头)和6个月时(C组,9头)行心脏彩色多普勒超声心动图、主动脉根部造影、实验室检查及安乐死后行解剖病理等检查,以评估XcorTM瓣膜置换效果。 结果 白猪平均体重(43.1±3.2)kg。XcorTM瓣膜系统在 20 头白猪中均成功植入,即刻器械成功率 100%。20 头白猪未发生不良事件和死亡。6个月后心脏彩色多普勒超声心动图检查显示:XcorTM瓣膜位置满意,开闭良好;人工瓣膜峰值跨瓣压差、瓣膜有效开口面积、峰值流速,主动脉瓣瓣环直径与术前相比差异有统计学意义(P<0.05);左室射血分数差异无统计学意义(P=0.733);实验室检查未见明显异常。大体病理学检查示白猪重要脏器无明显病理改变,组织病理学显示瓣叶无严重病理改变。 A、B、C三组主动脉瓣瓣叶钙盐含量分别为(0.32±0.22)μg/mg、(0.41±0.27)μg/mg 及(0.56±0.14)μg/mg,差异无统计学意义(P >0.05)。 结论 本动物实验显示使用XcorTM生物瓣膜行经心尖经导管主动脉瓣膜置换术是安全可行的,未发现与操作相关的并发症,可有效替代原生主动脉瓣正常工作。
Abstract:Objective To investigate the safety of XcorTM,a novel transcatheter heart valve system,in healthy white pigs.Methods A total of 20 healthy experimental white pigs were selected and implanted with XcorTM biological valves through transapical transcatheter aortic valve replacement. All subjects were randomly divided into three groups,and cardiac color Doppler echocardiography(CDE),aortic root angiography,laboratory examinations,and anatomic pathologypost?euthanasia were performed in animals immediately(group A,2),3 months(group B,9)and 6 months(group C,9)after surgery to evaluate the effect of XcorTM valve replacement. Results All average weight of white pigs was(43.1±3.2)kg. The XcorTM valve system was successfully implanted in all 20 white pigs,with a 100% success rate for immediateinstrumentation. There were no adverse events and no deaths among the 20 white pigs. Six months after operation,CDE showed the positions of XcorTM valves were satisfactory and the opening and closing were good. Compared with preoperative,the peak transvalvular pressure difference,the effective opening area of the valve,the peak flow velocity and aortic annular diameter were improved(P<0.05);while the left ventricular ejection fraction did not change significantly(P=0.733). Laboratory tests showed no significant abnormalities. Gross pathology showed no significant pathologic changes in the important organs of the white pigs,and histopathology showed no severe pathologic changes in the valve leaflets. The calcium content in the aortic valve leaflets of groups A,B,C was(0.32 ± 0.22)μg/mg,(0.41±0.27)μg/mg,and(0.56 ± 0.14)μg/mg,respectively. There was no statistically significant difference(P>0.05).Conclusion This animal study demonstrated that transapical transcatheter aortic valve replacement using the XcorTM bioprosthesis is safe and feasible,with no complications associated with the procedure,and that it can effectively replace the native aortic valve in normal operation.
