中文摘要: 目的 探讨改良体位在达芬奇Xi机器人胃癌手术中的应用效果。 方法 选择江苏省中医院2019年11月至2020年11月使用达芬奇Xi手术系统进行胃癌手术的患者114例,采用随机数字表法分成对照组57例和试验组57例,对照组采用常规人字体位。试验组采用改良体位。比较两组患者循环系统改变、体位摆放时长、术中不良事件发生情况。 结果 与对照组相比,试验组体位摆放时长缩短,医疗器械相关性压力性损伤、机械臂故障及静脉输液不畅率降低,手术医生对手术体位的满意度高,差异有统计学意义(P<0.05);两组患者体位安置及恢复前后收缩压(SBP)、舒张压(DBP)、心率(HR)改变比较差异无统计学意义(P>0.05)。 结论 机器人胃癌手术改良体位不影响患者循环稳定,利于节约操作时间,降低术中机械臂故障以及静脉输液不畅的发生率,减少医疗器械相关性压力性损伤的发生,提高护理质量及效率,增加患者及术者满意度,提高手术护理安全。
Abstract:Objective To investigate the application effect of improved body position in da Vinci Xi robot gastric cancer surgery. Methods A total of 114 patients who received gastric cancer surgery with da Vinci Xi robotic system in Jiangsu Province Hospital of Chinese Medicine from November 2019 to November 2020 were randomly divided into control group (n=57, herringbone split leg supine position) and experimental group (n=57, modified position). The changes of circulatory system, the length of position placement and the incidence of intraoperative adverse events were compared between the two groups. Results Compared with the control group, the length of position placement was shorter, the rate of pressure injury related to medical devices, mechanical arm failure and venous infusion obstruction were lower, and the satisfaction of surgeons with surgical position was higher in the experimental group (P<0.05). There was no significant difference in systolic blood pressure (SBP), diastolic blood pressure (DBP) and heart rate (HR) between the two groups before and after position placement and recovery (P>0.05). Conclusion The modified position of robotic gastric cancer surgery does not affect the patient's circulatory stability and it's conducive to saving operation time, reducing the incidence of intraoperative mechanical arm failure, poor intravenous infusion and the occurrence of medical device related stress injury. It can improve nursing quality and efficiency and increase the satisfaction of patients and surgeons.
