中文摘要: 目的 探讨星状神经节阻滞(SGB)对重症急性胰腺炎(SAP)患者脑氧代谢及炎性因子的影响。
方法 选取2016年6月至2018年12月河北北方学院附属第一医院重症医学科确诊SAP的患者21例,随机分为常规治疗组(对照组)12例和超声引导下SGB组9例,对照组予SAP的禁食禁水等常规治疗,SGB组在常规治疗基础上行超声引导下SGB。采集左侧桡动脉和右侧颈内静脉球部血于入室24 h(T0)、48 h(T1)、3 d(T2 )、7 d(T3)、14 d(T4)检测患者血流动力学和脑氧代谢的指标;采用ELISA法检测治疗前后患者血清IL-1β、IL-6、IL-10、TNF-α水平。
结果在脑氧代谢方面,两组患者动脉血氧含量(CaO2 )、动脉-颈内静脉球部血氧含量差(Da-jvO2 )、脑氧摄取率(CEO2 )水平随着时间推移呈下降趋势, 颈内静脉血氧含量(SjvO2 )呈升高趋势( P <0.01);CaO2 、Da-jvO2 、SjvO2 和CEO2 水平在两组间差异有统计学意义( P <0.01);两两比较,自T3时点起,SGB组的Da-jvO2 、CEO2 水平低于对照组,CaO2 、SjvO2 水平高于对照组( P <0.05, P <0.01)。在脑糖和乳酸代谢方面,两组患者动脉-颈内静脉球部血糖含量差(Da-jvGlu)水平随着时间推移下降,动脉-颈内静脉球部乳酸含量差(Da-jvLac)水平上升( P <0.01);Da-jvGlu和Da-jvLac水平在两组间差异亦有统计学意义( P <0.01);两两比较,自T2 时点起,SGB组的Da-jvGlu水平高于对照组,Da-jvLac水平低于对照组( P <0.05, P <0.01)。治疗结束后,SGB组中IL-1β、IL-6、IL-10、TNF-α水平均显著低于对照组( P <0.05, P <0.01)。
结论 SGB可通过减轻SAP患者应激反应所致CaO2 、SjvO2 、CEO2 、Da-jvO2 等指标的改变,改善其脑氧代谢;通过调控Da-jvLac、Da-jvGlu和炎性因子的水平,改善其炎症反应。
Abstract:Objective
To explore the effects of stellate ganglion block (SGB) on cerebral oxygen metabolism and inflammatory cytokines in the patient with severe acute pancreatitis(SAP).
Methods Twenty-one SAP patients in Department of Critical Care Medicine of the First Affiliated Hospital of Hebei North University from June 2016 to December 2018 were randomly divided into routine treatment group ( n =12,control group) and ultrasound-guided SGB combined with routine treatment group ( n =9,SGB group).The blood samples were collected from left radial artery and right internal jugular vein bulb for hemodynamics and oxygen metabolism monitoring at 24 h (T0), 48 h (T1), the third day (T2), the seventh day (T3) and the fourteenth day (T4) after admission. Enzyme-linked immunosorbent assay (ELISA) was used to detect the pre-and post-treatment serum levels of IL-1β,IL-6,IL-10 and tumor necrosis factor-α (TNF-α) .
Results
In cerebral oxygen metabolism, the levels of arterial oxygen content (CaO2 ), arterial internal jugular vein bulbar oxygen difference (Da-jvO2 ) and cerebral extraction of oxygen (CEO2 ) decreased over time, while jugular venous oxygen saturation (SjvO2 ) increased in two groups( P <0.01). There were significant differences in the levels of CaO2 , Da-jvO2 , SjvO2 and CEO2 between two groups ( P <0.01). From T3 time point,the levels of Da-jvO2 and CEO2 in SGB group were lower than those in control group, and the levels of CaO2 and SjvO2 were higher than those in control group by pairwise comparison ( P <0.05, P <0.01). In terms of cerebral glucose and lactic acid metabolism, Da-jvGlu level significantly decreased, Da-jvLac level increased over time in two groups( P <0.01),and there were statistic differences in them between two groups( P <0.01).From T2 time point with pairwise comparison, the level of Da-jvGlu in SGB group was higher than that in control group, and the level of Da-jvLac was lower than that in control group ( P <0.05, P <0.01). After treatment, the levels of IL-1β, IL-6, IL-10 and TNF-α in SGB group were significantly lower than those in control group ( P <0.05, P <0.01).
Conclusions
SGB can improve cerebral oxygen metabolism by reducing the changes of CaO2 , SjvO2 , CeO2 and Da-jvO2 caused by stress response, and improve the inflammatory response by regulating the levels of Da-jvLac, Da-jvgGlu and inflammatory factors in SAP patients.
