中文摘要: 目的 基于16S rDNA测序技术分析多囊卵巢综合征(PCOS)患者与健康女性肠道菌群的差异。方法 收集2019年10月至2020年9月就诊于山西省妇幼保健院门诊的初诊PCOS患者21例(PCOS组),同期行孕前检查或体检的健康女性20例(对照组)。入组对象均收集粪便及血液标本,粪便标本行高通量测序,血清用于性激素指标[促黄体生成素(LH)、促卵泡生成素(FSH)、雌二醇(E2)、睾酮(T)],糖代谢指标[空腹血糖(FPG)、空腹胰岛素(FINS)]测定,计算稳态模型胰岛素抵抗指数(HOMA-IR)。通过生物信息分析比较肠道菌群差异。结果 与对照组相比,PCOS患者血清FPG、FINS及HOMA-IR水平均升高(P<0.01);PCOS患者肠道菌群α多样性指数中Chao1指数较对照组增高,Shannon指数、Simpson指数较对照组降低,但差异无统计学意义(P>0.05)。两组在门、科、属水平上共同的优势菌为拟杆菌,门、科、属水平的拟杆菌丰度在PCOS组分别为21.93%、19.34%和19.34%,在对照组分别为10.02%、8.10%和8.10%,PCOS组三个水平的拟杆菌丰度均明显增加,且与LH/FSH比值呈正相关(r=0.493,P=0.023)。结论 PCOS患者存在明显糖代谢异常、肠道菌群紊乱。
Abstract:Objective Using 16S rDNA sequencing to analyze the difference of intestinal flora between patients with polycystic ovary syndrome (PCOS) and healthy women. Methods From October 2019 to September 2020, 21 PCOS patients (PCOS group) and 20 healthy women received pre-pregnancy examination or physical examination(control group) were selected. High-throughput sequencing was used to detected fecal samples. The serum levels of luteinizing hormone (LH), follicle stimulating hormone (FSH), estradiol (E2), testosterone (T), fasting plasma glucose (FPG) and fasting insulin(FINS) were determined, and homeostasis model assessment-insulin resistance (HOMA-IR) was calculated. The bioinformatics analysis was used to compare the differences of intestinal flora.
Results The levels of serum FPG, FINS and HOMA-IR in PCOS patients were higher than those in the control group (P<0.01). Compared with the control group, Chao1 index in PCOS patients increased, while Shannon index and Simpson index decreased, but the difference was not significant (P>0.05). Bacteroides were the common dominant bacteria in the phylum, family and genus levels of the two groups.The abundances of Bacteroides at phylum, family and genus were 21.93%,19.34% and 19.34% in PCOS group and 10.02%,8.10% and 8.10% in control group, respectively. The abundances of Bacteroidetes in PCOS group increased significantly and were positively correlated with the LH/FSH ratio(r=0.493,P=0.023). Conclusion PCOS patients have obvious abnormal glucose metabolism and intestinal flora disorder.
