中文摘要: 目的 探讨超声参数联合血清白细胞介素-17(IL-17)、可溶性白细胞介素-2受体(sIL-2R)、血管内皮生长因子(VEGF)在甲状腺癌临床诊断效能及与疾病病理特征的关联性。方法 选取2016年1月至2019年1月收治的甲状腺癌患者123例为恶性组,另选取同期123例甲状腺良性肿瘤患者为良性组。所有患者均接受超声检查及血清VEGF、IL-17、sIL-2R检测。对比两组患者超声参数［峰值强度（PI）、平均通过时间（MTT）、达峰时间（TTP）］和血清VEGF、IL-17、sIL-2R水平，以及上述指标在恶性组不同分期、淋巴结转移情况、分化程度患者中的差异；分析各指标与甲状腺癌病理特征关联性以及超声和血清指标单独与联合诊断的效能。结果 恶性组IL-17、VEGF水平高于良性组,sIL-2R、MTT、TTP、PI低于良性组(P<0.01);不同分期、不同淋巴结转移情况、不同分化程度甲状腺癌患者IL-17、VEGF、sIL-2R、MTT、TTP、PI间有统计学差异(P<0.01);IL-17、VEGF水平与肿瘤分期、淋巴结转移、分化程度间呈正相关关系,sIL-2R、MTT、TTP、PI与肿瘤分期、淋巴结转移、分化程度间呈负相关关系(P<0.01);联合诊断的AUC（0.920）大于IL-17、VEGF、sIL-2R、MTT、TTP、PI分别单一诊断的AUC（0.656~0.892）。结论 甲状腺癌患者血清VEGF、IL-17、sIL-2R水平及超声相关参数存在异常,各指标波动幅度与肿瘤分期、分化程度、淋巴结转移情况关系密切,临床可联合血清指标检测及超声检查对甲状腺癌进行鉴别诊断,为临床诊疗提供参考依据。
Abstract:ObjectiveTo investigate the diagnostic efficacy of ultrasound parameters combined with serum interleukin-17 (IL-17),soluble interleukin-2 receptor (sIL-2R) and vascular endothelial growth factor (VEGF) in thyroid cancer and their correlation with pathological characteristics.MethodsA total of 123 patients with thyroid cancer treated from January 2016 to January 2019 were selected as malignant group,and 123 patients with benign thyroid tumors were selected as benign group in the same period.All patients underwent ultrasound examination and serum VEGF,IL-17,sIL-2R detection.The ultrasound parameters［mean-transit time (MTT),peak intensity (PI),time to peak (TTP)］ and the levels of VEGF,IL-17 and sIL-2R were compared between two groups,and the differences of above indexes in patients with different stages, lymph node metastasis and differentiation in malignant group were analyzed.The diagnostic values of ultrasound and serum indicators,alone or in combination,on thyroid tumors were observed and analyzed.ResultsCompared with benign group,the levels of IL-17 and VEGF increased,and sIL-2R,MTT,TTP and PI decreased in malignant group（P＜0.01）.There were significantly differences in the levels of IL-17,VEGF,sIL-2R,MTT,TTP and PI of patients with different tumor stage,lymph node metastasis and tumor differentiation of thyroid cancer (P<0.01).The levels of IL-17 and VEGF were positively correlated with tumor staging,lymph node metastasis and differentiation degree,while sIL-2R,MTT,TTP and PI were negatively correlated with tumor staging,lymph node metastasis and differentiation degree (P<0.01).The AUC of the combined diagnosis(0.920) was greater than the AUC of the single diagnosis of IL-17,VEGF,sIL-2R,MTT,TTP,and PI(0.656-0.892),respectively.ConclusionIn patients with thyroid cancer,the serum VEGF,IL-17,sIL-2R levels and ultrasound parameters are closely related to tumor staging,differentiation degree and lymph node metastasis and can be used for differential diagnosis so as to provide reference for clinical diagnosis and treatment in clinical practice.
