Abstract:Objective To explore the effect of bilateral locking plate combined with calcium sulfate hemihydrate(CSH) bone graft on the treatment of complex tibial plateau fractures.? Methods Sixty-four patients with complicated tibial plateau fractures treated with bilateral locking plate combined with bone graft from February 2013 to February 2015 were retrospectively studied. All patients received anterolateral combined posteromedial incision approach. Progressive rehabilitation exercise was performed on the basis of fracture healing. In 1-, 3-, 6- and 12- month after operation, knee joint motion was measured by joint activity scale, and knee joint function was evaluated by hospital for special surgery (HSS) score. The tibial plateau angle (TPA) and posterior slope angle (PA) were evaluated by X-ray film immediately after operation and 3 -, 12- month after operation.? Results All patients were followed up for a mean duration of 12. 6 months (13-20 months). For all the patients, the flexion of keen joint was averagely at 90° (range from 80°-110°) in first postoperative month, at 115° (range from 90°-120°) in the 3th postoperative month, at 125° (range from 110°-130°) at the 6th postoperative months and at 135° (range from 120°-140°) in the 12th postoperative month. HSS scores of patients were respectively 55.4±3.2, 67.3±2.6, 76.3±1.2 and 90.6±2.4 at postoperative 1-, 3-, 6- and 12-month. There were no significant differences in TPA and PA between immediately after operation and postoperative 3 months and between postoperative 3 months and 12 months (all P>0.05). No complications, such as infection, loosening of internal fixation, necrosis of skin, and so on occurred in patients after operation.? Conclusion Bilateral locking plate and bone graft materials are effective and safe in the treatment of complex tibial plateau fractures, and early rehabilitation exercises can be achieved with satisfactory results and good recovery of knee joint function.
