中文摘要: 目的 分析类风湿关节炎(RA)患者血清25羟基维生素D[25(OH)D]、血浆白介素(IL)-6水平及临床意义。方法 选取2019年9月至2021年2月蚌埠医学院第一附属医院治疗的RA患者99例为研究组,按照28个关节疾病活动度(DAS28)评分分为:高度疾病活动期组36例,中度疾病活动期组25例,低度疾病活动期组21例,缓解期组17例。选取同期健康体检者68例作对照组。使用化学发光法检测定各组研究对象25(OH)D及IL-6水平。结果 对照组、缓解期组、低度活动期组、中度活动期组及高度活动期组25(OH)D水平依次降低,IL-6水平依次增高,各组间比较差异有统计学意义(P<0.05)。25(OH)D与IL-6水平、DSA28评分呈负相关(r=-0.746、-0.787,P<0.01);IL-6水平与DSA28评分呈正相关(r=0.886,P<0.01)。25(OH)D水平与D-二聚体(D-D)、红细胞沉降率(ESR)、C反应蛋白(CRP)、抗环瓜氨酸多肽(CCP)抗体、关节肿胀数(SJC)、关节压痛数(TJC)呈负相关(P<0.01)。IL-6水平与D-D、ESR、CRP、抗CCP抗体、SJC、TJC呈正相关(P<0.01)。结论 RA患者外周血25(OH)D水平降低,IL-6水平增高,二者联合监测可用以判断RA疾病活动程度。
Abstract:Objective To analyze serum 25-hydroxyvitamin D[25(OH)D] and plasma interleukin-6 (IL-6) levels in patients with rheumatoid arthritis (RA) and their clinical significance. Methods Ninety-nine RA patients treated in the First Affiliated Hospital of Bengbu Medical College from September 2019 to February 2021 were selected as the research group. According to the 28 joint disease activity (DAS28) scores, they were divided into high disease activity group (group H), moderate disease activity group (group M), low disease activity group (group L), and disease remission group (group R), 68 cases of healthy physical examination at the same period were selected as the control group. Chemiluminescence method was used to detect 25(OH)D and IL-6 levels in each group of subjects. Results The 25(OH)D levels of the control group, group R, group L, group M, and group H decreased in sequence, and the IL-6 level increased in sequence. The differences among the groups were statistically significant (P<0.05). 25(OH)D was significantly negatively correlated with IL-6 level and DSA28 score (r=-0.746, -0.787, P<0.01); IL-6 level was significantly positively correlated with DSA28 score (r=0.886, P<0.01). 25(OH)D level was significantly negatively correlated with D-dimer (D-D), erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), anti-cyclic citrullinated polypeptide (CCP) antibody, joint swelling number (SJC), joint tenderness number (TJC) (P<0.01). IL-6 levels was significantly positively correlated with D-D, ESR, CRP, anti-CCP antibodies, SJC and TJC (P<0.01). Conclusion The level of 25(OH)D in the peripheral blood of RA patients decreases, and the level of IL-6 increases. The combined monitoring of the two can make a more reliable clinical judgment on the degree of RA disease activity.
