中文摘要: 目的 探讨全关节镜下腘窝囊肿切除术对于治疗腘窝囊肿患者的临床疗效及复发率。
方法 对2017年3月至2019年3月进行手术治疗的148例腘窝囊肿患者的临床资料进行回顾性分析,按照所采用的治疗方式分为关节镜组与传统手术组各74例。其中关节镜组采用全关节镜下腘窝囊肿切除术,传统手术组采用传统手术治疗。随后统计患者手术相关指标(切口长度、住院时间、术中出血以及手术时间),比较两组患者手术前后Rauschning-Lindgren分级、膝关节功能，随访囊肿复发率情况。
结果 关节镜组患者切口长度、住院时间与传统手术组比较明显缩短(P均<0.01),术中出血量明显降低(P<0.01),但手术时间与传统手术组比较明显延长(P<0.01);术后,关节镜组与传统手术组患者Rauschning-Lindgren分级结果均明显改善,且关节镜组优于传统手术组(P<0.01);术后两组患者膝关节功能评分均升高,且关节镜组高于传统手术组(P<0.01)。术后随访5~28(14.36±5.24)个月,随访期间关节镜组术后有3例(4.05%)复发,传统手术组患者术后有11例(14.86%)复发,两组患者术后复发率比较差异有统计学意义(P<0.05)。
结论 对腘窝囊肿患者应用全关节镜下腘窝囊肿切除术的临床疗效显著,不但能够有效缩短住院时间,促进术后恢复,同时还能有效改善患者膝关节功能,降低术后复发率。
Abstract:Objective To investigate the clinical efficacy and recurrence rate of popliteal cyst resection with total arthroscopy for popliteal cyst.
Methods The clinical data of 148 patients with popliteal cysts who underwent surgical treatment from March 2017 to March 2019 were retrospectively analyzed,and then the cases were divided into arthroscopy group and traditional operation group (n=74,each) according to the treatment method.The arthoscopy group underwent total arthroscopic popliteal cyst resection and the traditional operation group underwent traditional surgery.Subsequently,the patient′s surgery-related indicators (incision length,length of hospital stay,intraoperative bleeding and operation time) were counted,and the Rauschning-Lindgren classification,knee joint function in two groups were compared before and after surgery,and the recurrence rate of cysts was followed up.
Results The incision length and length of hospital stay in the arthoscopy group were significantly shorter than those in the traditional operation group (all P<0.01),and the intraoperative blood loss was significantly lower than that in the traditional operation group (P<0.01),but the operation time was significantly longer than that in the traditional operation group (P<0.01).After surgery,the Rauschning-Lindgren classification results in two groups were significantly improved,and was better in the arthoscopy group than that in the traditional operation group (P<0.01);the knee function scores of two groups were increased,and was higher in the arthoscopy group than that in the traditional operation group (P<0.05);During the follow-up period of 5-28 (14.36 ± 5.24) months,3 cases (4.05%) relapsed in the arthoscopy group,11 cases (14.86%) relapsed in the traditional operation group,and there was a significant difference in recurrence rate between the two groups(P<0.05).
Conclusion The application of total arthroscopy for popliteal cyst resection in patients with popliteal cysts has a significant clinical effect,which can not only effectively shorten the length of hospital stay and promote postoperative recovery,but also effectively improve the knee joint function and reduce the postoperative recurrence rate.
