2型糖尿病肾病患者血清GDF-15与hs-CRP、IL-17、Cys-C、mAlb的相关性

中文摘要: 目的 检测2型糖尿病肾病患者血清生长分化因子-15(GDF-15)水平,并对GDF-15与超敏C-反应蛋白(hs-CRP)、白细胞介素-17A(IL-17A)等炎症指标的相关性进行分析,探讨GDF-15在2型糖尿病肾病中的临床意义。方法 选取2015年至2016年144例糖尿病肾病患者,包括大量白蛋白尿组(26例)、微量白蛋白尿组(60例)和正常白蛋白尿组(58例)。采用免疫比浊法测定各组血清中胱抑素C(Cys-C)、hs-CRP浓度,ELISA法检测GDF-15、IL-17A浓度,并分析血清GDF-15与各项指标的相关性。结果 A组糖尿病病程和GDF-15、IL-17A、Cys-C、hs-CRP水平高于B组和C组(P<0.05),B组和C组之间比较无统计学差异(P>0.05)。偏相关分析显示,GDF-15与尿微量白蛋白(mAlb)(r=0.280,P<0.01)、Cys-C(r=0.184,P<0.01)呈正相关,与糖尿病病程(r=0.097,P>0.05)、hs-CRP(r=0.147,P>0.05)、IL-17A(r=-0.660,P>0.05)无相关。结论 GDF-15与2型糖尿病肾病的发生与发展密切相关。
Abstract:Objective To assess growth differentiation factor-15(GDF-15) status in patients with type 2 diabetic nephropathy (DN) and its relation to inflammatory parameters,such as high sensitive C-reactive protein (hs-CRP),interleukin-17A (IL-17A). Methods A total of 144 DN patients treated from 2015 to 2016 were selected and divided into macroalbuminuria group (group A,n=26),microalbuminuria group (group B,n=60) and normal albuminuria group (group C,n=58) according to urinary protein content.The concentrations of cystatin C (Cys-C) and hs-CRP in serum were detected by immunoturbidimetry,and GDF-15 and IL-17A were detected by ELISA.The associations of GDF-15 with various indexes were analyzed. Results The course of diabetes,the levels of GDF-15,IL-17A,Cys-C and hs-CRP in group A were significantly higher than those in group B and group C (all P<0.05),and there were no significant differences in them between group B and group C (P>0.05).Partial correlation analysis showed that GDF-15 was positively correlated with urinary microalbumin (mAlb) (r=0.280,P<0.01),Cys-C (r=0.184,P<0.01),but not correlated with the course of diabetes (r=0.097,P>0.05),hs-CRP (r=0.147,P>0.05),IL-17A (r=-0.660,P>0.05). ConclusionGDF-15 is closely related to the occurrence and development of type 2 diabetic nephropathy.
