SDF-1 / CXCR4轴在溃疡性结肠炎病理过程中的作用

中文摘要: 目的 探讨基质细胞衍生因子1(SDF-1)/CXC超化因子受体(CXCR4)轴在溃疡性结肠炎(UC)病理过程中的作用。方法 选取2016年3月至2017年9月消化内科诊治的UC患者67例,另选取同期健康体检者60例作为对照。酶联免疫吸附法(ELISA)检测外周血SDF-1水平,流式细胞仪检测外周血CD4+T淋巴细胞表面CXCR4的表达水平及表达CXCR4的CD4+T淋巴细胞(CD4+CXCR4+)的比例。结果 UC患者外周血的SDF-1水平和CD4+CXCR4+T淋巴细胞比例分别为(25.63±4.18)μmol/L和(9.44±1.62)%,均分别高于对照组的(7.24±2.36)μmol/L和(2.62±0.51)%(P<0.05)。活动期UC患者SDF-1水平和CD4+CXCR4+T淋巴细胞比例分别为(28.92±4.64)μmol/L和(10.45±1.76)%,均分别高于缓解期的(15.16±3.07)μmol/L和(6.23±1.18)%(P<0.01)。活动期UC患者不同内镜分级间SDF-1水平和CD4+CXCR4+T淋巴细胞比例存在显著性差异(P<0.05),其中SDF-1水平和CD4+CXCR4+T淋巴细胞比例在Ⅲ级患者显著高于Ⅱ级患者(P<0.05),Ⅱ级患者又显著高于Ⅰ级患者(P<0.05)。结论 SDF-1/CXCR4轴激活在UC发病机制中发挥着重要作用,有可能作为患者病情活动性以及肠道病变严重程度的监控指标。
Abstract:Objective To investigate the role of stromal cell derived factor-1(SDF-1) / CXC chemokine receptor 4 (CXCR4) axis in the pathological process of ulcerative colitis(UC). Methods Sixty-seven UC patients and 60 health examiner in Huangshi Central Hospital from March 2016 to September 2017 were selected. Enzyme-linked immunosorbent assay (ELISA) was used to detect SDF-1 level in peripheral blood. Flow cytometry was used to detect expression of CXCR4+ in CD4+ T lymphocyte surface and the proportion of CD4+ CXCR4+ T lymphocytes with positive expression of CXCR4+. Results SDF-1 level [(25.63±4.18)μmol/L vs (7.24±2.36)μmol/L, P<0.05] and the proportion of CD4+ CXCR4+ T lymphocytes [(9.44±1.62)% vs (2.62±0.51)%, P<0.05] in UC patients were significantly higher than those in control group. SDF-1 level [(28.92±4.64)μmol/L vs(15.16±3.07)μmol/L, P<0.01] and the proportion of CD4+ CXCR4+ T lymphocytes [(10.45±1.76)% vs (6.23±1.18)%, P<0.01] in UC patients of active stage were significantly higher than those in UC patients of remission stage. There were significant differences in SDF-1 level and the proportion of CD4+ CXCR4+ T lymphocytes for different endoscopic classification in UC patients of active stage (all P<0.05) which were higher in class Ⅲ patients than those in class Ⅱ patients (P<0.05) and in class Ⅱ patients than those in class Ⅰ patients (P<0.05). Conclusion The activation of SDF-1/CXCR4 axis plays an important role in the pathogenesis of UC, and it is possibly served as a monitoring indicator for the activity of the disease and the severity of the bowel disease.
